Pseudomonas aeruginosa Infections of Intact Skin

Abstract: Pseudomonas aeruginosa infections of healthy skin are uncommon. We report four cases of P. aeruginosa infections of intact skin. These cases illustrate the clinical spectrum of these cutaneous infections: localized, mild epidermal infections (the green nail syndrome and webbed space infections), moderately serious infections (cutaneous folliculitis and otitis externa), and, in immunocompromised patients, extremely serious infections (malignant otitis externa, perirectal infection, and ecthyma gangrenosum).

“…The patients were evaluated after 6 weeks of application of 0.1% octenidine dihydrochloride solution with microscopy and culture. Patients having both negative microscopy and culture results were considered as cured regardless of nail coloring, since the nail hue has been suggested to result from diffusion of pyocyanin rather than the true invasion of Pseudomonas in nail plate [1] .…”

“…It is considered to be a complication of onycholysis of various origin or chronic paronychia [2] , and this is confirmed in our series ( table 1 ). It also seems that a paronychial infection with Pseudomonas is usually preceded by onycholysis or paronychia, leading to pyocyanin pigment staining of the adjacent nail [1] . The disease commonly is restricted to 1 or 2 nails.…”

“…The most often isolated bacteria in infected external auditory canals are P. aeruginosa and Staphylococcus spp. [Brook et al, 1992;Agger and Mardan, 1995;Amigot et al, 2003;Schaefer and Baugh, 2012;Lee et al, 2013]. These bacteria could not be cultured in any swabs after placement of the Lyric2 device in our study.…”

“…Pseudomonas aeruginosa may be presented with mild cutaneous infections in healthy persons like a toe web infections, hot tub folliculitis, however, in persons with lowered resistance, more severe infections such as malignant otitis externa and necrotizing fasciitis are observed [1]. Ecthyma gangrenosum (EG), the characteristic skin symptom of Pseudomonas septicemia, occurs in debilitated or in immunocompromised patients [2,3].…”
