Raised dermal scars including hypertrophic, and keloid scars as well as scalp‐associated fibrosing Folliculitis Keloidalis Nuchae (FKN) are a group of fibrotic raised dermal lesions that mostly occur following cutaneous injury. They are characterized by increased extracellular matrix (ECM) deposition, primarily excessive collagen type 1 production by hyperproliferative fibroblasts. The extent of ECM deposition is thought to be proportional to the severity of local skin inflammation leading to excessive fibrosis of the dermis. Due to a lack of suitable study models, therapy for raised dermal scars remains ill‐defined. Immune cells and their associated markers have been strongly associated with dermal fibrosis. Therefore, modulation of the immune system and use of anti‐inflammatory cytokines are of potential interest in the management of dermal fibrosis. In this review, we will discuss the importance of immune factors in the pathogenesis of raised dermal scarring. The aim here is to provide an up‐to‐date comprehensive review of the literature, from PubMed, Scopus, and other relevant search engines in order to describe the known immunological factors associated with raised dermal scarring. The importance of immune cells including mast cells, macrophages, lymphocytes, and relevant molecules such as cytokines, chemokines, and growth factors, antibodies, transcription factors, and other immune‐associated molecules as well as tissue lymphoid aggregates identified within raised dermal scars will be presented. A growing body of evidence points to a shift from proinflammatory Th1 response to regulatory/anti‐inflammatory Th2 response being associated with the development of fibrogenesis in raised dermal scarring. In summary, a better understanding of immune cells and associated molecular markers in dermal fibrosis will likely enable future development of potential immune‐modulated therapeutic, diagnostic, and theranostic targets in raised dermal scarring.

The article shows the key role of IL-17 in the psoriasis pathogenesis and the opportunities of its management via monoclonal antibodies product secukinumab. The review of international randomized trials on clinical efficacy and safety of genetically engineered biologic drug secukinumab in children and adolescents with psoriasis is presented. During treatment periods of 12 to 52 weeks, secukinumab has shown high therapeutic efficacy for psoriasis severity and skin lesion areas and has improved quality of life of children and adolescents according to dynamic assessments of PASI, IGA 0/1 mod 2011 indices, CDLQI questionnaire. The safety profile of secukinumab in children is estimated as favorable and comparable to using it in adults.

The relevance of this topic is determined by the widespread prevalence of dermatosis in the population, the growth of its severe forms, and diagnostic difficulties due to the clinical features of the course of psoriasis in children. This often requires a histological examination of the skin, even despite the age of children. Recently, according to our observations, Devergie's disease has become one of the most frequent dermatoses with which differential diagnosis of psoriasis has to be carried out. This case is given of a clinical observation of dermatosis in a 13-year-old girl, in which the disease at the begining of development was similar to the manifestations of Devergie's disease, and the results of a study of a biopsy of the affected skin did not exclude this diagnosis. A possible reason could be the effect of steroid therapy on the clinical and morphological manifestations of dermatosis. Subsequent dynamic observation and repeated histological examination allowed us to make a diagnosis of psoriasis. Modern data on the prevalence of psoriasis in children are presented, and the features of the course of dermatosis in children are highlighted. In pediatric practice, difficulties arise with the choice of external therapy. The clinical results of the use of various forms of zinc pyrithione activated in the treatment of psoriasis are presented.
