Pseudoaneurysm of the Radial Artery After a Bicycle Fall

Abstract: We report a case of a 64-year-old man who developed a painful pulsatile mass in the distal forearm after a bicycle fall with fracture of the wrist. Ultrasonography confirmed a 2.5-cm large pseudoaneurysm of the radial artery. The patient underwent surgical exploration. The pseudoaneurysm was resected and the defect in the arterial wall was reconstructed with an autologous saphenous vein patch. We suggest that the double arterial supply of the hand should be preserved whenever possible.

“…It is necessary to perform an Allen test as part of the preoperative workup. Surgical treatment with revascularization seems to be the best approach [ 20 ] and has a high success rate with minimal morbidity. Nevertheless, simple ligature is possible depending on the location of the aneurysm.…”

“…Non‐iatrogenic trauma is considered an uncommon cause of pseudoaneurysms, especially in relation to the radial artery. A literature search of the PubMed database from the past 10 years confirms the infrequency of this pathology 1,5 . Of the 76 cases identified, 60 were attributed to iatrogenic trauma (79%), 12 cases to non‐iatrogenic trauma (16%) and four cases to idiopathic causes in patients with connective tissue disorders (5%).…”

“…Pseudoaneurysms are rare occurrences in peripheral arteries, with non‐iatrogenic causes described infrequently 1 . When left untreated, pseudoaneurysms may develop into a visible, pulsatile mass.…”
