Pruritus and quality of life in moderate‐to‐severe plaque psoriasis: post hoc explorative analysis from the PRISTINE study

Abstract: Most patients with moderate-to-severe psoriasis in this study (96%) reported pruritus. Pruritus improved significantly with ETN therapy and was strongly associated with improvements in QoL. These data support the clinical relevance of pruritus as an important symptom of patients with moderate/severe psoriasis.

“…Of the group of dermatoses, it is of interest that psoriasis was searched for as frequently as atopic dermatitis, once more supporting the role of pruritus in this disease. Although a neglected area of medicine, recent studies have demonstrated the frequency and burden of psoriatic itch . Interestingly, certain localizations were more prominent than others.…”

“…On questionnaires, patients have often labelled itch in psoriasis as ‘bothersome’, ‘annoying’, ‘unbearable’ and ‘worrisome’ . In fact, clinical evidence has shown that itching can significantly impair health‐related quality of life, with itch severity correlating well with the magnitude of impairment . Many aspects of psychosocial well‐being are affected by itch.…”

“…Dealing with itch can lead to increased feelings of agitation, embarrassment, and stigmatization and contribute to stress‐related disorders, anxiety and depression . Mood, concentration, sexual desire and appetite are also affected by the severity of itch . Patients with itchy skin will have disrupted sleep patterns that can lead to itch‐related sleeping disorders, with a consequent negative impact on work productivity through impairment in activity functions .…”

“…One study documented the efficacy of narrowband ultraviolet B (UVB) therapy in treating psoriatic itch. Biologics, like etanercept, may also be of value to reduce pruritus intensity (Mrowietz et al, 2015). However, in some patients, narrowband UVB may aggravate itch by increasing skin dryness, and UVB therapy should be supplemented with moisturizers or emollients (Dawn and Yosipovitch, 2006).…”
