Proximal Tubules Have the Capacity to Regulate Uptake of Albumin

Abstract: Evidence from multiple studies supports the concept that both glomerular filtration and proximal tubule (PT) reclamation affect urinary albumin excretion rate. To better understand these roles of glomerular filtration and PT uptake, we investigated these processes in two distinct animal models. In a rat model of acute exogenous albumin overload, we quantified glomerular sieving coefficients (GSC) and PT uptake of Texas Red-labeled rat serum albumin using two-photon intravital microscopy. No change in GSC was o… Show more

“…Intravital MPM has helped expand the investigative focus, beyond glomerular dysfunction, to elucidate the role tubular injury plays in proteinuric and albuminuric diseases that were previously thought to be associated solely with damage to the filtration barrier. In this section we will outline methods to study both pathways [8, 9]. In quantifying uptake, it is important not to saturate the intensity of the endosomal pool (particularly lysosomes) as this will underestimate the accumulation of the material therein [12].…”

“…Previous studies relied on following the folate receptor due to the inability to fix folate in place for subsequent visualization via immunolocalization techniques [2]. Some of these discoveries have challenged long established dogma as disruptive technologies often do [38]. …”

“…Substantial data from multiple laboratories, using different techniques and models, are either ignored or rigorously refuted rather than used as the basis to rethink new possibilities and implications and move the field forward. 41,42 Although progress in transplantation and some glomerular diseases is reasonable, in other areas such as AKI and diabetic and nondiabetic CKD the clinical progress has been extremely slow despite the availability of a considerable amount of government and industry funding and a solid base of preclinical data. This lack of progress has often been blamed on inadequate preclinical models and many other factors; however, I believe we are stuck without the necessary ability and technologies to translate these findings to the clinic.…”

“…5E). Since the renal tubular epithelia are shown to modulate proteinuria through tubular reabsorption of proteins in urine (28), knockdown of lrp2a, encoding the megalin transporter in renal epithelial cells, revealed a significant increase in proteinuria in Tg(pod: Gal4 high ;UAS:CA-RAC1,cmlc2:EGFP) transgenic fish, confirming that the GFB in these fish was defective even without induction of podocyte injury (Fig. 5E).…”
