Proximal Interphalangeal Joint Arthroplasty Using the Wide-Awake Local Anesthesia No Tourniquet Technique

Abstract: Background: Proximal interphalangeal joint (PIPJ) osteoarthritis is a common condition that results in pain, stiffness, and loss of function in the affected hand. Proximal interphalangeal joint arthroplasty is an effective treatment option when conservative methods have failed. The wide-awake local anesthesia no tourniquet (WALANT) technique to perform surgery carries advantages such as lack of tourniquet discomfort, reduces the staffing and costs associated with anesthesia and sedation, and allows faster reco… Show more

“…Эндопротезирование проксимального межфалангового сустава остается нерешенной биомеханической проблемой, несмотря на достижения в области материалов и новых конструкций имплантатов [21,22]. Целями этой операции являются уменьшение болевого синдрома, увеличение объема движений, восстановление биологической оси пальцев кисти и улучшение их функции [23,24].…”

“…WALANT may help with intraoperative release of adduction contracture or the stabilization of metacarpophalangeal instability if a swan-neck deformity persists during surgery (67). Reconstruction of interphalangeal joints with WALANT can provide active testing of the implants (68).…”

“…Protocols for the use of wide-awake local anaesthesia no tourniquet (WALANT) in upper limb surgery have expanded and gained ground in recent years. Initially described by Lalonde as an anesthetic alternative for repairing flexor tendon injuries of the fingers, nowadays it is used for procedures of increasing complexity in the upper limb [1][2][3][4][5][6][7].…”
