Proximal gastrectomy versus total gastrectomy for adenocarcinoma of the esophagogastric junction: a meta-analysis

Abstract: Aim: To compare efficacy between total gastrectomy (TG) and proximal gastrectomy (PG) for upper-third gastric cancer. Materials & methods: PubMed, Embase and Cochrane library were searched to select suitable researches. Stata was used for meta-analysis including 5-year overall survival rate, recurrence rate, complication morbidities and serum nutritional levels. Results: Ten retrospective English researches were contained. Our study showed no significant difference of 5-year overall survival rate, recurren… Show more

“…Concerning neo-/adjuvant therapy, an essential factor for survival, there was no significant difference between the 2 groups. In addition, our study also revealed TG enabled a more completed nodal dissection, which was concordance with all published studies [47, 48]. The extent of lymphadenectomy was another important consideration when TG and PG are compared, especially the dissection of #4d, #5, and #6, which are usually excluded in PG.…”

“…Recently, postoperative quality of life has received significant attention in addition to oncological outcomes. This was mainly because the incidence of postgastrectomy disturbances in patients who underwent PG was high as previously reported, including some meta-analyses [17, 29, 47, 48, 50]. In our experience, the PG group showed a higher rate of anastomotic stenosis and reflux symptoms but similar reflux esophagitis when compared with TG.…”

“…While the feasibility and safety of these two methods have been demonstrated in a variety of studies, most of them were singlecenter studies with small sample sizes and limited follow-up periods. Previous meta-analysis relied on data mostly obtained from conventional open surgery (23)(24)(25)(26)(27). To overcome these limitations, a meta-analysis of studies comparing LPG and LTG for proximal gastric cancer should be performed.…”

“…Some authors assume that SG may be beneficial and preferable for proximal GC [10,11]. Goto et al reported noninferiority of SG for remnant GC at stage IA disease [12].…”
