Proximal Femoral Screw Hemiepiphysiodesis in Children With Cerebral Palsy Improves the Radiographic Measures of Hip Subluxation

Abstract: Background: Proximal femoral screw hemiepiphysiodesis (PFSH) is a promising technique for treatment of hip subluxation in children with cerebral palsy (CP). The aim of this study is to report radiographic outcomes of PFSH and to evaluate its role in changing the natural history of hip displacement in children with CP. Methods: This is a single center retrospective chart review of children with CP that underwent PFSH with at least 2 years of radiographic follow-up. Demographic information, surgical details, c… Show more

“…Shedding light on this subject was a paper published out of Cincinnati Children's Hospital with a 2-year follow-up of 44 hips that underwent proximal femoral screw hemiepiphysiodesis. 23 In this group, those that previously had surgery had no progression of their hip subluxation. There were improvements in migration percentage in the children who did not have prior hip reconstructive surgery; however, four hips did undergo reconstructive hip surgery in this group after their proximal femoral hemiepiphysiodesis.…”

“…Researchers from Cincinnati (Ohio, USA) performed a retrospective review of the records of children with CP undergoing PFSH, and who had at least two years of radiological follow-up in their centre. 8 Radiographs were assessed for a range of relevant measurements including head shaft angle (HSA), neck shaft angle (NSA), and migration percentage (MP) as well as details of the surgery, complications, additional procedures, and the requirement for screw exchange. Some patients in the series had undergone previous hip reconstruction, and so the cohort was divided into groups according to the presence or absence of this previous intervention prior to PFSH treatment.…”
