Despite the willingness, humans belong to the species with the limited procreation possibilities. Infertility affects about 15 % of population is the very important subject for the reproductive medicine. An assisted reproductive technology (ART) offers the significant chance for the infertile couples, but it does not give the guarantee for the pregnancy and for the birth of a healthy child. The implantation of embryo, despite numerous trials and attempts, remain the last barrier in the assisted reproduction technologies; thus, the endometrial receptivity becomes the subject of permanent interest. In this review we have tried to present various methods of improvement of an endometrial receptivity with the conclusion that we still wait for the valuable prognostic factor in the treatment of infertility by ART which could predict the chance for the birth of a healthy child.

Infertility is a significant demographic and socio-economic problem, which is solved with assisted reproductive technologies. Relevance of the infertility problem continues to grow, which leads to the search for new methods to improve the success of assisted reproductive technologies. Purpose — to analyze the effect of anthropometric factors, obstetrical and gynaecological history on the risk of implantation failure in women with infertility who have fertilization in vitro. Materials and methods. An analysis of 106 infertile women who were treated with assisted reproductive technologies in September 2019 — February 2021. The control group consisted of women who received infertility treatment according to the standard treatment regimens: controlled ovarian stimulation, gamete sampling, intracytoplasmic sperm injection and embryo transfer. The study group included patients who received antiplatelet agents, anticoagulants and intravenous immunoglobulins G in addition to the standard therapy. Results. In this study, no differences were found in the two study groups regarding age, height, weight, body mass index, menarche, menstrual duration, coitus (p>0.05). During the analysis, it was found that with increasing age of a woman, the risk of unsuccessful implantation increases, OR=1.11 (95% CI 1.01–1.21) for each year, with increasing patient height, the risk decreases (p=0.006), OR=0.85 (95% CI 0.76–0.96) for each centimetre, with an increase in the number of sexual intercourses per week (p=0.012), OR=1.71 (95% CI 1.12–2.60) the risk increases by each unit. Implantation success with using modified treatment regimens with additional aspirin, low molecular weight heparins, intravenous immunoglobulins G, was higher (p=0.049), OR=0.37 (95% CI 0.14–0.99) compared to the standard infertility treatment scheme. Conclusions. This study found unfavourable effects of age, woman's height, and sexual intercourses on embryo implantation. Additional use of aspirin, low molecular weight heparin, intravenous immunoglobulin G as a supplemental therapy to standard treatment regimens of infertility with IVF had a successful impact on the effectiveness of embryo implantation. The research was carried out in accordance with the principles of the Helsinki declaration. The study protocol was approved by the Local Ethics Committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interest was declared by the authors. Background Infertility remains a significant public health concern. An issue with controlled ovarian stimulation (COS) is the selection of an exogenous gonadotropin (Gn) regimen, which is mainly based on urinary follicle-stimulating hormone (uFSH), recombinant follicle-stimulating hormone alfa (rFSH-alfa), and human menopausal gonadotropin (HMG). In addition, most previous studies focused on the clinical pregnancy rates or live birth rates (LBR) per transfer cycle, but not on the cumulative live birth rate (CLBR) per started cycle. The CLBR, appears to be a more comprehensive and accurate universal measure of IVF treatment success. Therefore, this study aimed to compare the cumulative live birth rate (CLBR) between rFSH-alfa and uFSH regimens for ovarian stimulation. Methods This retrospective cohort study included patients who underwent assisted reproductive technology (ART) with gonadotropin-releasing hormone (GnRH) agonist long protocol between March 2009 and December 2018. Patients were grouped according to the Gn regimen received (rFSH-alfa or uFSH). The main outcome was CLBR, which defined as the first live birth following the use of all fresh and frozen embryos derived from a single COS cycle. Results A total of 1078 cycles were analyzed (314 with rFSH-alfa and 764 with uFSH). The rFSH-alfa group was characterized by a higher number of retrieved oocytes (13.3 vs. 11.0) and transferable embryos (5.0 vs. 4.0), a higher fresh embryo transfer rate (35.0% vs. 26.3%), and a higher multiple birth rate among the fresh embryo transfer cycles (8.2% vs. 2.5%) (P < 0.05). There were no differences in pregnancy rate (32.7% vs. 33.8%) and LBR (25.5% vs. 26.9%) per transfer cycle (P > 0.05). No significant difference was found in clinical outcomes among the frozen embryo transfer cycles (P > 0.05). The CLBR per started cycle in the rFSH-alfa group was higher than in the uFSH group (53.5% vs. 43.1%, P < 0.05). After adjustment, rFSH-alfa was independently associated with a higher CLBR (OR = 1.56; 95%CI = 1.18–2.05; P = 0.0018). Conclusions rFSH-alfa and uFSH have similar pregnancy rates and LBR per transfer cycle, rFSH-alfa might achieve more transferrable blastocysts and higher CLBR per started cycle compared to uFSH.
