Proton Pump Inhibitor Therapy for Suspected GERD-Related Chronic Laryngitis: A Meta-Analysis of Randomized Controlled Trials

Abstract: PPI therapy may offer a modest, but nonsignificant, clinical benefit over placebo in suspected GERD-related CL. Validated diagnostic guidelines may facilitate the recognition of those patients most likely to respond favorably to PPI treatment.

“…A subsequent meta-analysis of the placebo-controlled studies in laryngopharyngeal reflux has reported minimal therapeutic benefit of PPIs over placebo. 76 GERD has also been shown to be present in 50-80% of asthmatic patients. 77,78 Reports of GERD-related symptoms in adults with asthma range from 65% to 77%.…”

“…Th e experience with treating laryngeal symptoms attributed to refl ux disease is comparable. A meta-analysis of eight randomized controlled trials found that PPI therapy had no signifi cant advantage over placebo in achieving improvement of symptoms of suspected GERD-related chronic laryngitis (RR 1.28, 95 % CI 0.94 to 1.74) ( 141 ).…”

“…Laryngoscopy and monitoring pH are the methods used for the diagnosis of LPR (13), though their real specificity and sensitivity values have been interrogated (18). Traditionally, successful empirical treatment with proton pump inhibitors is regarded as a diagnostic method (10); however this method has its limitations and is also questioned by the literature (18,19). In this study, the criteria used to diagnose "reflux laryngitis disease" were the following laryngoscopic findings: arytenoid hyperemia and laryngeal mucosa and the vocal folds edema (posterior laryngitis).…”
