Proton pump inhibitor-responsive esophageal eosinophilia

Abstract: Proton pump inhibitor-responsive esophageal eosinophilia is an inappropriate disease descriptor, arbitrarily based on a response to a single drug, and should be abandoned. Patients who have esophageal eosinophilia and esophageal symptoms that resolve with PPI therapy have phenotypic, molecular, mechanistic, and therapeutic features indistinguishable from similar patients who do not respond to PPIs. These patients with PPI responsiveness should be considered within the spectrum of EoE.

“…Our study highlights the similarities and differences between the patients with final diagnoses of EoE vs. PPI-REE. While it has been recently postulated that EoE and PPI-REE are the same entity ( 21 ), our study did identify distinctions between the two populations. While severe clinical findings such as history of dysphagia or food impaction did not significantly differ between the two groups, this may be a consequence of this being a pediatric study and these symptoms occur more frequently in adults.…”

“…In the case that eosinophilic inflammation subsided, we would be facing a PPI-REE. This name, according to experts, should be abandoned as it is an inadequate descriptor of this pathology ( 46 , 47 ). The main novelty of the new guidelines on EoE ( 9 ) is the retraction of the term “PPI-REE,” considering PPIs are at the same level as diets or topical steroids in the treatment of EoE and are not a diagnostic criteria anymore.…”

“…Of the 22 studies selected from European centers, 12 studies (55%) considered values within the range 11 to ≤15 eosinophils/hpf 17, 36, 37, 38, 39, 46, 47, 53, 54, 55, 56, 57, 58 as histological remission, and 4 studies (23%) defined histological remission as less than 5 or ≤ 5 eosinophils/hpf 30, 33, 42, 45. Strikingly, all studies conducted in Switzerland used a more stringent and consistent end‐point of <5 eosinophils /hpf to identify cases of histological remission 28, 59, 60…”
