Purpose: To evaluate the effect of an age-dependent D-Dimer cut-off in patients who underwent a computed tomography pulmonary angiogram (CTPA) for suspected pulmonary embolism (PE) Material and Methods: Retrospective application of an age-dependent D-dimer cut-off (age/100 in patients aged over 50) in 530 consecutive patients, both in- and outpatients, aged over 18, who underwent CTPA for suspected PE according to the guidelines. Results: The application of an age-dependent D-dimer cut-off showed a now negative test-result in 17 of 530 patients (3.2?%). The proportion was 4.1?% (17 of 418) in patients aged over 50. None of these 17 cases was diagnosed with PE in CTPA, the false-negative rate was 0?%. The effect could be seen in outpatients (14 of 377 [3.7?%]) as well as in inpatients(3 of 153 [2.0?%]) with no statistically significant difference (p?>?0.05). Conclusion: The application of an age-dependent D-dimer cut-off as part of the guidline-based algorithm for suspected PE reduced the number of necessary CTPA in outpatients as well as in inpatients. Radiotherapy for organs with respiratory motion has motivated the development of dynamic volume lung imaging with computed tomography (4D-CT) or magnetic resonance imaging (4D-MRI). 4D-CT can be realized in helical (continuous couch translation during image acquisition) or cine mode (translation step-by-step), either acquired prospectively or reconstructed retrospectively with temporal resolutions of up to 250 msec. Long exposure times result in high radiation dose and restrict 4D-CT to specific indications (ie, radiotherapy planning). Dynamic MRI accelerated by parallel imaging and echo sharing reaches temporal resolutions of up to 10 images/sec (2Dþt) or 1 volume/s (3Dþt) that allow analyzing respiratory motion of the lung and its tumors. Near isotropic 4D-MRI can be used to assess tumor displacement, chest wall invasion, and segmental respiratory mechanics. Limited temporal resolution of dynamic volume acquisitions (in their current implementation) may lead to an overestimation of tumor size, as the mass is volume averaged into many voxels during motion. Nevertheless, 4D-MRI allows for repeated and prolonged measurements without radiation exposure and therefore appears to be appropriate for patient selection in motion-adapted radiotherapy as well as for a broad spectrum of scientific applications.

How to deal with solitary pulmonary nodules (SPN) which are incidentally detected by computed tomography (CT) is an increasingly important task in the era of modern multislice CT. This paper reviews the morphological and functional characteristics and their value for discrimination between benign and malignant SPNs. In particular, the importance of nodule size, growth rate, margin morphology, density, calcifications or fatty components within the nodules, the significance of cavitations or aerobronchograms, enhancement patterns at dynamic contrast-enhanced CT and findings on positron emission tomography (PET) are discussed. The Bayesian analysis to calculate the probability of malignancy is presented. Finally, flow charts demonstrate the national and international recommendations for nodule management.
