Introduction Tobacco-free nicotine pouches are products that are placed between the lip and gum, where they deliver nicotine to users. Little is known about nicotine pouch use in Great Britain since they entered the market in 2019. Methods Data came from a monthly representative survey of the adult (≥18y) population in Great Britain (England, Scotland, and Wales) between November 2020 and October 2021 (n=25,698). We estimated the weighted prevalence of pouch use, overall and stratified by demographics, smoking status, and other nicotine use. Results Nicotine pouch use was rare among adults, with a weighted prevalence of just 0.26% (95% compatibility interval [CI]=0.19-0.35). Prevalence doubled from November 2020 to October 2021 (0.14% to 0.32%; Prevalence ratio [PR]=2.22, 95%CI=1.33-3.70). Pouch use was four times more common among men than women (0.42% versus 0.09%; PR=4.55, 95%CI=2.27-9.09) but less common in older age groups (p<.001). Pouch use was more prevalent among current smokers (0.87%; PR=13.60, 95%CI=5.46-33.89), recent former smokers (0.97%; PR=15.21, 95%CI=4.03-57.42), and long-term (>1y) former smokers (0.24%; PR=3.71, 95%CI=1.36-10.15), compared with never smokers (0.06%). Prevalence was also elevated among e-cigarette (1.64% versus 0.15%; PR=10.59, 95%CI=5.74-19.52) and nicotine replacement therapy users (2.02% versus 0.21%; PR=9.75, 95%CI=4.64-20.49). Conclusions One in 400 adults in Great Britain use nicotine pouches, but prevalence increased from 2020 to 2021. Implications Tobacco-free nicotine pouches were introduced to the market in Great Britain in 2019. We found that, while pouch use is currently rare in Great Britain, these products have become more popular over time. Pouch use is largely concentrated among younger and middle-aged men who use other nicotine products and have a history of smoking. Continued monitoring of nicotine pouch use is needed.

The World Health Organization estimates there are 1.1 billion cigarette smokers across the globe and that tobacco related deaths number 7 million per year. Electronic nicotine delivery systems (ENDS) are available to contribute options for smoking cessation and include e-cigarettes, e-hookahs, vape pens, mods, and vaping. The growing use of ENDS, or e-cigarettes, in the US and globally across populations is dramatic. Although users may think that e-cigarettes are less harmful than combustible tobacco products, the evidence shows that there are known risks and harms for users. E-cigarettes have varying amounts of toxicants, nicotine, and carcinogens and put the user at risk for lung diseases and COVID-19 similar to smokers. Currently, most governing bodies have not approved e-cigarettes as a smoking cessation tool but do state if a person has failed conventional smoking cessation treatments that e-cigarettes used alone for the short term may help those to quit combustible tobacco and nicotine. A shared decision-making approach should be used when discussing e-cigarettes as a harm reduction tool. More studies and long-term data are needed to assess potential benefits and harms. What is known is that prevention efforts and policy are needed to avoid adolescents and other vulnerable populations from initiating tobacco or e-cigarette use.

Context: To examine the level of support for tobacco availability policies across Great Britain (GB) and associations between support for policy and sociodemographic, smoking and quitting characteristics. Methods: Cross-sectional data came from a representative survey of adults in GB (n = 2,197) during September 2021. Weighted proportions indicated the level of support for each policy. Logistic regressions estimated the associations between support for each policy and sociodemographic and smoking characteristics. Findings: There was majority support for requiring retailers to have a license which can be removed if they sell to those under-age (89.6% (95%CI 88.3–90.9)) and for restrictions on the sale of cigarettes and tobacco near schools (69.9% (67.9–71.9)). More supported than opposed raising the legal age of sale of cigarettes and tobacco to 21 (49.2% supported; 30.7% opposed; 20.1% unsure) and reducing the number of retailers selling tobacco in neighbourhoods with a high density of tobacco retailers (46.5% supported; 23.3% opposed; 30.2% unsure). More opposed than supported a ban on the sale of cigarettes and tobacco to everyone born after a certain year from 2030 onwards (41.3% opposed; 34.5% supported; 24.2% unsure). Age was positively associated with support for raising the age of sale (OR 1.06 95%CI 1.01–1.12), and inversely associated with requiring tobacco retailer licenses (OR 0.86, 0.79–0.94).Women were more likely to support raising the age of sale (OR 1.25, 1.05–1.49) and reducing the number of retailers (OR 1.34, 1.12–1.60). Current smokers were less likely to support proposed availability policies than never smokers. Smokers with high motivation to stop had higher odds of supporting a ban on tobacco sales (OR 3.45, 1.68–7.14) and reducing the density of retailers (OR 2.60, 1.29–5.27). Conclusions: Requiring tobacco retailer licensing and restrictions on sales near schools received majority support. Other tobacco availability policies received substantial support despite considerable opposition. Many of the public remain unsure. Policy Points • Strong majority support for tobacco retailer licensing and restrictions on sales near schools suggests that implementation of these tobacco availability policies would be popular among adults in Great Britain. • There was greater support than opposition to raising the legal age of sale of tobacco to 21 and reducing the number of retailers in high density tobacco retailer neighbourhoods. • Opposition to a ban on tobacco sales has weakened compared with a previous survey in England, and many of the public remain unsure; informed debate will be critical in determining public attitudes towards this tobacco availability policy.
