“…This intervention uses an established chronic care model to redesign hypertension health services, train clinical staff, and engage patients and the community. The multi-interventional program will address many of the issues identified in this review as contributing to therapeutic inertia and poor adherence in Colombia, including active patient follow-up, standardized treatment algorithms, patient education by clinic staff and peers (specially trained “expert hypertension patients”), training to improve physician–patient communication, and availability of core antihypertensive medications through a central pharmacy [ 75 ].
