Актуальність. Необхідність ідентифікувати, лікувати, запобігати тяжким порушенням нутритивного балансу у пацієнтів з політравмою залишається одним з основних компонентів інтенсивної терапії. Правильний вибір часу та методу нутритивної терапії, складу та енергетичної цінності харчових сумішей покликаний сприяти зниженню частоти важко коригованих ускладнень, в тому числі синдрому поліорганної недостатності. Мета: оцінити ефективність раннього ентерального харчування з розширеним білково-калорійним забезпеченням згідно з протоколом Enhanced Protein-Energy Provision via the Enteral Route Feeding Protocol in Critically III Patients (PEP uP) у комплексі інтенсивної терапії пацієнтів з тяжкою політравмою. Матеріали та методи. Обстежено 40 хворих із тяжкою політравмою, доставлених у відділення анестезії та інтенсивної терапії (ВАІТ) в середньому через 0,5 години після травми. Середній вік постраждалих-41,9 ± 2,4 року. Основна група (група А)-15 пацієнтів, яким проводилась нутритивна терапія згідно з протоколом PEP uP. Групу порівняння (група Б) становили 25 хворих, у яких нутритивна підтримка проводилася відповідно до європейських рекомендацій з клінічного харчування пацієнтів у ВАІТ із застосуванням стандартних полімерних сумішей. З метою оцінки ризику поліорганної недостатності застосовували шкалу Sequential Organ Failure Assessment (SOFA) при надходженні, на 2-гу, 3-тю та 5-ту добу лікування. Визначали в динаміці рівень загального білка та альбуміну сироватки крові. Результати. Тяжкість органної дисфункції за шкалою SOFA в обох групах при госпіталізації була однорідною. В групі Б з 2-ї доби відзначено розвиток стійкої гіпопротеїнемії, чого в групі А не спостерігалось. На 5-ту добу лікування в групі А виявлено вірогідно вищий рівень альбуміну, ніж у групі Б.

Acute liver failure (ALF) is a rare syndrome resulting from an acute insult to the liver in patients without known underlying chronic liver disease. It is characterized by loss of synthetic function in the form of jaundice and coagulopathy and development of hepatic encephalopathy. Multiorgan failure (MOF) eventually develops, leading to death. Many different etiologies have been identified, with acetaminophen (APAP) overdose and viral hepatitis being the most common causes worldwide. The pathophysiology of ALF can be divided into cause-specific liver injury pathophysiologies and pathophysiology related to occurrence of secondary MOF. In terms of liver injury pathophysiology, APAP toxicity is the most well known. Secondary MOF is often a result of the initial massive proinflammatory response generating a systemic inflammatory response syndrome followed by a compensatory anti-inflammatory response leading to immune cell dysfunction and sepsis. As the liver is a tremendously important metabolic organ involved in energy metabolism, protein synthesis, fat metabolism, and glycemic control, multiple aspects of nutrition also need to be considered as part of the overall pathophysiology of ALF. Aim: The aim of this work is to develop a pragmatic approach in the assessment and management strategies of patients with cirrhosis in order to optimize the outcomes in this patient population. Method: A systematic review of literature was conducted through 8 July 2017 on the PubMed Database looking for key terms, such as malnutrition, nutrition, assessment, treatment, and cirrhosis. Articles and studies looking at associations between nutrition and cirrhosis were reviewed. Results: An assessment of malnutrition should be conducted in two stages: the first, to identify patients at risk for malnutrition based on the severity of liver disease, and the second, to perform a complete multidisciplinary nutritional evaluation of these patients. Optimal management of malnutrition should focus on meeting recommended daily goals for caloric intake and inclusion of various nutrients in the diet. The nutritional goals should be pursued by encouraging and increasing oral intake or using other measures, such as oral supplementation, enteral nutrition, or parenteral nutrition. Conclusions: Although these strategies to improve nutritional support have been well established, current literature on the topic is limited in scope. Further research should be implemented to test if this enhanced approach is effective.
