Protective effect of PCV vaccine against experimental pneumococcal challenge in adults is primarily mediated by controlling colonisation density

Abstract: Widespread use of Pneumococcal Conjugate Vaccines (PCV) has reduced vaccine-type nasopharyngeal colonisation and invasive pneumococcal disease. In a double-blind, randomised controlled trial using the Experimental Human Pneumococcal Challenge (EHPC) model, PCV-13 (Prevenar-13) conferred 78% protection against colonisation acquisition and reduced bacterial intensity (AUC) as measured by classical culture. We used a multiplex qPCR assay targeting lytA and pneumococcal serotype 6A/B … Show more

“…Following exposure, 41 volunteers remained culture-negative (41/63, 65.1%) and 22 became culture-positive (see Table S1 in the supplemental material). Colonization status was confirmed by S. pneumoniae 6A/B capsule-specific and lytA -specific quantitative PCR (qPCR) in nasal wash to ensure that we did not miss low-density colonizers ( 18 ). Only one culture-negative volunteer showed a low positive signal (threshold cycle [ C T ] = 37) at day 6 post S. pneumoniae exposure.…”

“…[34] In an experimental human pneumococcal challenge model, adults who received PCV13 had lower pneumococcal density compared with those who received a control vaccine. [37] To date, there is not consistent evidence of an effect of PCV on pneumococcal density in children. Variations in density observed in cross-sectional studies with carriage assessed during different years may be temporal and/or related to as number of unmeasured factors.…”

“…Those that do, have heterogeneous findings. A double-blind, randomized controlled trial of PCV13 and Hepatitis A vaccine (control arm) in adults, using the Experimental Human Pneumococcal Challenge model, found that pneumococcal density in the PCV13 arm was significantly lower compared with the control arm (P<0.0001) [60]. In a cluster-randomized trial of PCV7 and Meningococcal C vaccine (control arm) in rural Gambia, density of PCV7 pneumococcal carriage was lower in PCV7 villages compared with control arm villages [61].…”
