Protection by remote ischemic preconditioning during coronary artery bypass graft surgery with isoflurane but not propofol – a clinical trial

Abstract: Thus, RIPC during isoflurane but not during propofol anesthesia decreased myocardial damage in patients undergoing CABG surgery. Accordingly, effects of RIPC evoked by upper limb ischemia/reperfusion depend on background anesthesia, with combined RIPC/isoflurane exerting greater beneficial effects under conditions studied.

“…We became aware in our initial studies that propofol interferes with RIPC25, 26 and have since performed all our studies with isoflurane anesthesia. The RIPC protocol consisted of 3 cycles of 5‐minute left upper arm ischemia (by inflation of a blood pressure cuff to 200 mm Hg)/5 min reperfusion (cuff deflated).…”

“…Omitting the 4 studies using another protocol than the mainly used 3×5-min remote conditioning protocol from analysis, barely affected the heterogeneity (I 2 =68%, P=0.0009), and changed the pooled SMD in favor of RIC (-0.46, 95% CI: -0.76 to -0.16) in the Characteristic a Kottenberg et al 24 Lucchinetti et al 25 Hong et al 26 Target Figure 4B). 6,7,17,20,21, 23 Sensitivity analysis showed that the omission of each study one at a time did not appreciably change the pooled SMD, and the estimates in each case were within the CI of the pooled estimate ( Figure 5).…”

“…a Kottenberg et al allocated patients into 4 groups receiving either isoflurane or propofol anesthesia with or without remote ischemic preconditioning and presented data separately. 24 The effect sizes of the CABG studies are plotted against their precision, which is the inverse of standard error. Black boxes, original studies relating remote ischemic conditioning to biomarkers of myocardial injury; black diamond, standardized mean difference (SMD) and 95% confidence interval (95% CI) for the metaanalysis.…”

“…34 This could disrupt pharmacologic recruitment of cardioprotective signal transduction. 35,36 Detoxification of ROS by phenolic antioxidants involves conversion to peroxide and a phenoxyl radical that stimulates DNA fragmentation. 12 Furthermore, propofol induced transcriptional changes in fatty acid metabolism and DNA damage signalling, correlating with release of N-terminal pro-brain natriuretic peptide (NT-proBNP), a biomarker of cardiac dysfunction.…”
