Prostatectomía simple (adenomectomía) por vía laparoscópica: experiencia en 59 pacientes consecutivos

Abstract: Laparoscopic extraperitoneal simple prostatectomy is an effective procedure for the treatment of large prostatic adenomas. There seems to be less perioperative morbidity and, in our experience, this technique seems to be feasible and reproducible; however, its learning curve is a complicated matter to deal with.

“…The transperitoneal technique was used by some investigators [ 11 , 13 , 17 ], but is associated with a risk of ileus, peritonitis, and bowel injuries along with urine leakage and possible urine peritonitis from the bladder suture. No studies have compared extra– and transperitoneal laparoscopic adenomectomy in the way laparoscopic or robotic radical prostatectomy has been compared [ 24 26 ]. However, in LA, these two approaches give comparable results ( Table 1 ).…”

“…The limitations of LSP are previous pelvic surgery and known bleeding diathesis in which case HoLEP is preferable [7] . Also, only experienced laparoscopic surgeon can perform LSP with a favourable outcome [24] .…”

“…A potential stiff learning curve for the majority of urologists could be considered an additional drawback of LSP. [ 15 ] Handling of the large adenoma during enucleation could be sometimes difficult with the mass often interfering with the operating field. An excellent collaboration with the first assistant is mandatory while separation of prostate into its lobes and extracting each of them after enucleation could preserve space within the restricted prostatic cavum.…”
