Prospects in non-invasive assessment of liver fibrosis: Liquid biopsy as the future gold standard?

Abstract: Liver fibrosis is the result of persistent liver injury, and is characterized by sustained scar formation and disruption of the normal liver architecture. The extent of fibrosis is considered as an important prognostic factor for the patient outcome, as an absence of (early) treatment can lead to the development of liver cirrhosis and hepatocellular carcinoma. Till date, the most sensitive and specific way for the diagnosis and staging of liver fibrosis remains liver biopsy, an invasive diagnostic tool, which … Show more

“…1 This accumulation can in turn cause distortion of the normal hepatic architecture and ultimately progress to cirrhosis, which is irreversible and causes impaired liver function. 2,3 Noninvasive tests for the diagnosis of liver fibrosis have been developed. Although many direct and indirect surrogate markers for cirrhosis have been suggested, no single surrogate marker is used widely in clinical practice.…”

“…The implication of these circulating factors to predict liver brosis is becoming more popular because they are available in many laboratories, they are almost non expensive, and can be repeated several times. Moreover, they may be representative of the brosis in the whole liver, thus avoiding small sampling error which is a technical defect in percutaneous liver biopsy [21].…”

“…Such invasive procedures are associated with certain complications. The majority of patients refuse to undergo liver biopsy, and they are even more reluctant to accept a second liver biopsy, making it difficult to assess the efficacy of the procedure (14). Additionally, sample errors, and intraobserver or interobserver differences may affect the accuracy of the diagnostic results (15)(16)(17).…”
