Prospective Study on the Treatment of Lower-Extremity Chronic Venous and Mixed Ulcers Using Tissue-Engineered Skin Substitute Made by the Self-assembly Approach

Abstract: The authors' study suggests that the SASS used as a biological dressing is a promising treatment for hard-to-heal chronic venous and mixed ulcers that are unresponsive to compression therapy.

“…7,12 Briefly, fibroblasts were first expanded for 7 days in DMEM 10% FCS. After expansion (7 days), fibroblasts were seeded in flasks (Falcon) at 4 X 10 3 cells/cm 2 and cultured for 25 days in DMEM 10% FCS 50 µg/ml ascorbic acid.…”

“…Our group previously presented a bilayered self-assembled skin substitute (SASS) made of a dermal substitute underlying a fully differentiated epidermis, which generated an engineered tissue similar in structure and function to native human skin [4][5][6] for clinical use in wound healing 7 and burn treatment. 8,9 SASS is currently used in a clinical trial for the permanent coverage of full-thickness wounds (ClinicalTrials.gov NCT02350205).…”

“…The skin substitute was produced by the self-assembly method, an approach of tissue engineering based on the formation of ECM-rich cell sheets through the stimulation of fibroblasts with ascorbic acid 34 . It allows the production of human skin substitutes (epidermis and dermis) whose clinical relevance has already been well demonstrated by our group 35,36 . We incorporated human microvascular endothelial cells to recapitulate the formation of blood and lymphatic capillaries in the dermal compartment, and then melanoma spheroids were added to the tissue.…”

“…We have developed a new way to obtain a very dense tissue using only cells that is based on the capability of cells to produce and organize their own extracellular matrix. This method is called the self-assembly method and has been used to reconstruct skin [5,6], adipose tissue [7] and blood vessels [8]. However, production of these tissues is a lengthy process, which is a major drawback to using this method to reconstitute tissues or organs and quickly treat patients at low cost.…”
