Prospective randomized single-center, single-blind Comparison of laparoscopic vs open repair of pediatric inguinal hernia

Abstract: As compared with IH patients who undergo open surgery, those who have a laparoscopic repair suffer less pain, and their recovery and wound cosmesis are more satisfactory. With LR, contralateral hernias can be detected and repaired in a single operative procedure. This procedure takes slightly longer for unilateral than for bilateral hernias.

“…62,63 A prospective, randomized, single-blinded trial comparing laparoscopic to open repair of inguinal hernias showed that children who were older than 3 months of age when laparoscopic repair was performed required significantly fewer doses of pain medication. 64 The utility of laparoscopic repair of inguinal hernias in younger infants remains undetermined to date. …”

“…But they further suggested that considering low incidence of metachronicity in unilateral hernias and the relatively less significance of cosmetic outcome over groin region, conventional open herniotomy may continue as the standard of care especially in resource constrained centres of the developing world. 11,12 As regards the complications, in our study, reactive hydrocele was noticed in 22 cases and all of them resolved by second week. Hydrocele during postoperative period is a frequent occurrence and indicates the continuing fluid secretion from the left over distal sac.…”

“…Many studies by other authors have compared the operating time of laparoscopic procedure with open procedure and reported shorter time with laparoscopy specially in bilateral cases [11,13]. However, time taken will depend on many factors including experience of surgeon and few authors have reported longer time with laparoscopy [16,17]. Since assessment of postoperative pain is difficult in patients below 2 years of age, only patients above 2 years were evaluated in this study.…”
