Prospective Longitudinal Study of Gait and Balance in a Cohort of Elderly Essential Tremor Patients

Abstract: Background: Essential tremor (ET) encompasses a variety of features, including tremor, cognitive dysfunction, and gait and balance impairments. Gait and balance impairments in ET are often mild, but they can be severe and are, in some cases, associated with functional sequelae in terms of increased fall risk and reduced balance confidence. Previous research on gait and balance in ET has been limited to cross-sectional comparisons. There have been no longitudinal studies or prospective studies. As su… Show more

“…First, many ET cases are elderly, and they do not use ethanol. Studies show that as few of 19% 35 to 42.9% 36 to 67% 37 use ethanol. Second, response to ethanol is not specific to ET, being a feature of many tremor disorders (e.g., primary writing tremor, orthostatic tremor, isolated vocal tremor, and Parkinson disease) and a large number of other movement disorders (e.g., torticollis, spasmodic dysphonia, and generalized dystonia), 38,39 both because of its calming effects and likely also because of direct effects.…”

“…Changes in mobility-related parameters can be used to measure the effect of treatment [8,9] and to track disease progression [10,11]. This information can be potentially used to provide a more individualized treatment.…”

“…Since disequilibrium is also a commonly reported ADR of primidone, it may be speculated that ET patients with mild ataxia, when exposed to primidone, may develop sudden worsening of the ataxia. It needs to be emphasized that such gait abnormalities are not uncommon in older ET patients and their progression may be related to poor cognition [47,48]. However, there are no data at present to support this speculation.…”
