Prospective Evaluation of Endoscopic Ultrasonography and Endoscopic Retrograde Cholangiopancreatography in Patients with Chronic Abdominal Pain of Suspected Pancreatic Origin

Abstract: Twenty asymptomatic paid volunteers (13 females, 7 males, mean age 34 years, range 21-52 years), underwent endoscopic ultrasonography (EUS) to assess variation in the appearance of ductular and parenchymal features of the pancreas. Following this investigation, 69 patients with chronic abdominal pain of suspected pancreaticobiliary origin were evaluated with EUS followed by endoscopic retrograde cholangiopancreatography (ERCP) and in 16 patients secretin stimulated intraductal pure pancreatic juice (PPJ) colle… Show more

“…The hypothesis that all these criteria have the same value for the diagnosis was proposed in the Rosemont classification (4), which defines the EUSbased criteria for CP and their specific validity, establishing different patients groups. However, the interobserver variation of this classification does not improve the aforementioned (3)(4)(5).…”

“…The activation of pancreatic stellate cells produces parenchymal fibrosis and progressive loss of acinar cells and, to a lesser extent, of the islets of Langerhans (1)(2)(3)(4)(5). Endoscopic ultrasound (EUS) has become the technique of choice for the diagnosis of pancreatic disease such as CP, especially in early stages, in which other methods do not achieve the same accuracy (1)(2)(3)(4)(5)(6).…”

“…Previous reports suggest that EUS can be a sensitive modality for diagnosing chronic pancreatitis when compared to ERCP findings [7,9,10,12,25]. However, currently inter observer variation might affect EUS diagnosis of chronic pancreatitis.…”

“…Endoscopic ultrasonography (EUS) is a well-established and less-invasive modality for CP diagnosis [5][6][7][8][9][10]. Although endoscopic retrograde pancreatigraphy (ERP) has been regarded as the gold standard for CP diagnosis, ERP cannot investigate the parenchymal changes.…”
