Prospective Comparison of Magnetic Resonance Imaging, Transrectal and Transperineal Sonography, and Surgical Findings in Complicated Perianal Crohn Disease

Abstract: Transperineal sonography is a very accurate diagnostic method with outstanding sensitivity compared with MRI and transrectal sonography for evaluation of complicated perianal Crohn disease. Due to its simplicity and low cost, it is recommended that transperineal sonography be the first diagnostic modality in these cases.

“…Several studies suggest that PNUS should be used as a preliminary tool in patients with suspected perianal fistulas and abscesses 120 121 122 123 . PNUS is also useful for monitoring patients with perianal fistulizing Crohn's disease under treatment with biologics.…”

“…All modalities have been shown to correctly classify Crohn’s perianal fistulae in over 85% of patients and when two modalities are used, the accuracy approaches 100%[ 14 ]. Transperineal ultrasound, which is a non-invasive and relatively cheaper modality of imaging, has also been shown to have similar accuracy to TRUS and MRI pelvis in diagnosing perianal fistulae[ 15 ]. If available, pelvic MRI provides the most comprehensive evaluation of the perianal region (Figure 1 ).…”

“…However, although it was first described some years ago, it is only performed in a few centres. Bor et al [ 10 ] stated that for patients with PCD, the accuracy of transperineal US is identical to that of MRI or endoanal sonography.…”
