Prospective comparison of endosonography, magnetic resonance imaging and surgical findings in anorectal fistula and abscess complicating Crohn's disease

Abstract: AES with a linear probe is more accurate than MRI in detecting anorectal abscesses complicating Crohn's disease, and much more accurate in the evaluation of complex fistulas.

“…In the general context of pelvic sepsis and anorectal fistulae complicating CD, anal endosonography has been reported as the best modality in 1 study. 38 However, state of the art comparative data is clearly warranted in CD. The lack of a need to introduce an endoluminal device, unlimited view and perfect comparison between different examinations, with less dependence on a specially skilled operator favors MR, anal endosonography being a good alternative.…”

“…[12][13][14] The use of either technique is therefore determined by availability, expertise and preferences. In general, EUS can be used when low, simple tracks are expected, while MRI can be used in any patient and especially when the presence of complex fistula is suspected.…”

“…Estimates of the sensitivity and specificity for detecting abscesses have been reported in a somewhat higher range, i.e. between 71 and 100 for the sensitivity and between 77 and 94% for the specificity [27,[29][30][31][32] .…”
