Propranolol Accelerates Adipogenesis in Hemangioma Stem Cells and Causes Apoptosis of Hemangioma Endothelial Cells

Abstract: Background-Infantile hemangiomas (IHs) can cause significant morbidity during proliferation, yet there is no FDA-approved treatment. IHs are believed to form from stem cells (HemSCs), which differentiate towards an endothelial cell (HemECs) phenotype. Recently, propranolol has demonstrated effectiveness in the treatment of complicated IHs. We hypothesize that propranolol facilitates IH involution by altering cellular behavior in both HemECs and HemSCs.

“…HemSC isolation and characterization were done as previously described (51,52). In brief, proliferating regions of IH tissue were digested with collagenase, and HemSCs were isolated by CD133 + magnetic bead selection (Miltenyi Biotech).…”

“…NCr nude females (strain Foxn1nu) 4-6 weeks old were used (Taconic). The xenograft IH mouse model was performed as previously described (51,52). On day 0, 1.5 × 10 6 HemSCs suspended in 200 μl of Matrigel (Corning) were implanted subcutaneously into 7-to 9-week-old female NCr nude females (Taconic).…”

“…The mechanisms whereby propranolol works include vasoconstriction of the high-flow blood vessels feeding the hemangioma, suppression/blockade of VEGF and bFGF with induction of apoptosis of capillary endothelial cells, blockade of GLUT1 receptors, and accelerated adipogenesis of hemangioma stem cells. 9,33,34 Other authors suggest that propranolol does not induce apoptosis of infantile hemangioma stem cells, which is in contrast with the result of capillary endothelial cells. 35 In contrast to corticosteroids, propranolol is effective in treating hemangiomas in children of all ages, and not limited to the proliferative stage of the lesions.…”

“…12,13,[20][21][22] Suggested therapeutic mechanism of propranolol was inhibition of angiogenesis and induction of apoptosis in endothelial cells. 23 Propranolol has been used for hemangiomas in our clinic since 2009. Herein we presented the treatment experience with oral propranolol in 27 patients with hemangioma.…”
