Propofol-Induced Changes in Myoplasmic Calcium Concentrations in Cultured Human Skeletal Muscles from RYR1 Mutation Carriers

Abstract: Malignant hyperthermia is a pharmacogenetic disorder caused by autosomal dominant mutations in the ryanodine receptor type 1 gene. Propofol has been reported as a safe anaesthetic for malignant hyperthermia susceptible patients but has not been tested on cultured cells from patients with the ryanodine receptor type 1 mutation. The aim of this study was to determine whether propofol could trigger abnormal calcium fluxes in human myotubes isolated from malignant hyperthermia susceptible patients harbouring the n… Show more

“…In Dainese et al [ 15 ], we also showed that incidence of mortality in CASQ1-null mice during exposure to both halothane and heat is significantly greater in males than in females, a finding in line with some epidemiological studies conducted in humans, which reported a male prevalence of ~3 : 1 to 4 : 1 [ 25 27 ]. To date, the reason for this gender difference in humans and in CASQ1-null mice remains unclear.…”

“…Thirty-four patients were enrolled in this study (Table 1), including 17 individuals described in a previous study 17,18 . Four patients (numbers 5, 6, 15 and 16) experienced MH events diagnosed as rank 5 or 6 according to clinical grading scale 19 during general anaesthesia.…”

“…The CICR rate was measured according to Endo's method [15][16][17][18] . In brief, bundles of skeletal muscle fibres 5 to 6 mm in width and 10 to 15 mm in length were dissected into smaller pieces, and these pieces were isolated in the relaxing solution under a stereomicroscope.…”

“…Case no. 2 had a C>T point mutation in RYR1 exon 47 at position 7522 and the pathologic diagnosis was myopathic changes with some fibres exhibiting cores or core-like structures, type 1 fibre predominance and type 2b fibredeficiency 10,17 . The non-accelerated group did not include individuals with muscle disease or anyone who reported a previous MH episode.…”

“…However, its safety has not been confirmed by studies at the human cellular level on Ca 2+ homeostasis. Recently, we examined the effects of propofol on Ca 2+ homeostasis in isolated human myotubes from two patients carrying RYR1 mutations linked to MH 17 . With the addition of more cases, the aim of this study was to investigate the safety of propofol and its mode of action upon skeletal muscle cells.…”
