\s=b\ Biocompatible Teflon fluorocarbon polymer (Proplast) and porous polyethylene (Plasti-Pore) are porous alloplastic implant materials that are widely used in reconstructive head and neck surgery. These two materials were used in our study as replacement grafts for defects in three groups of 12 canines. For the first time, Plasti-Pore was found equal and perhaps superior to Proplast in deep implantation, this in the anterior cricoid cartilage when the internal and external perichondrium were preserved. Rejection of both substances occurred when the implant was exposed to air and aerodigestive tract contaminants after the removal of the perichondrium, subglottic, and upper tracheal mucosa. Airway exposure to the alloplasts, even when lined with buccal mucosa, resulted in infection and extrusion, but at a much slower rate.

Acquired laryngotracheal stenosis is a well‐documented complication of endotracheal intubation. Severe acquired laryngotracheal stenosis in children responds to surgical reconstruction, including the use of grafts and stents. The most commonly described graft material is autogenous cartilage, placed in the anterior laryngotracheal complex. The role of autogenous cartilaginous grafts sewn between the divided lamina of the posterior cricoid cartilage is less well established. Several authors report good clinical results using autogenous cartilaginous grafts to the posterior larynx; the single reported animal study of grafts to the posterior larynx failed to show significant benefit. This study was undertaken to examine the hypothesis that there is no difference in survival of anteriorly placed or posteriorly placed cartilaginous grafts in the larynx of growing rabbits, and to report on 61 pediatric patients with laryngotracheal stenosis treated with autogenous cartilaginous grafts placed between the vertically divided posterior lamina of the cricoid cartilage. The animal study showed that posteriorly placed grafts survived with viability equal to or better than that of anteriorly placed grafts. The clinical study showed that 59 of the 61 patients were decannulated with satisfactory vocal quality, good exercise tolerance and sleep patterns, and with no mortality.

The variety of methods used for the treatment of severe tracheal stenosis and the occasional failure to obtain lasting and consistent relief are measures of the difficulty of the problem. Presently, several surgical approaches are used for reconstructing large defects of the cervical trachea, reminding us that no single technique has gained wide clinical acceptance. We resected a U-shaped cartilage from 6 to 8 rings of the trachea of 10 New Zealand rabbits. We used a porous high-density polyethylene (PHDP) prosthesis to reconstruct the cervical trachea, and did not perform a tracheotomy during this operation. We followed up the rabbits for 4 to 10 months. One of the rabbits died in the second month because of respiratory distress; another was lost in the fourth month of the study while delivering young. The others continued to thrive; in fact, one of them gave birth 3 times after our study and was well able to take care of her young. At the end of the study, the rabbits were painlessly sacrificed in order to remove the larynx and trachea and examine them histopathologically. In the histopathologic examinations, the prostheses were incorporated into the native trachea and adhered to the surrounding organs, especially to the esophagus. No mucosal irregularities were seen on the surface of the prosthesis, and all the surfaces appeared to be covered with ciliated pseudostratified epithelium. This tracheal prosthesis provides good results in rabbit tracheal reconstruction, and appears very promising for the clinical repair of tracheal defects.
