Prophylactic endoscopic sclerotherapy of oesophageal varices in liver cirrhosis. A multicentre prospective controlled randomised trial in Vienna.

Abstract: SUMMARYThe effect of prophylactic treatment of oesophageal varices by endoscopic injection sclerotherapy before the first episode of variceal haemorrhage was studied in patients with cirrhosis in a prospective, randomised and controlled multicentre trial. From February 1984 to March 1987 patients with liver cirrhosis and large varices (stage III-IV according to Paquet) were treated and followed up. The sample comprised 87 patients: 45 in the prophylactic treatment and 42 in the control group. After excluding d… Show more

“…Various sclerosants were used at different doses and injected intra or paravariceally. The results of these trials are variable with two studies showing a significant reduction in both bleeding and mortality,54 55 one study showing a reduction in mortality but no difference in rebleeding,62 one study showing a significant increase in the risk of bleeding,59 and another significantly greater mortality 65. Due to the marked heterogeneity between these studies a meta-analysis is clinically inappropriate.…”

“…Endoscopic re‐treatments are available and effective in many of the patients, who eventually have recurrent diseases 55 . However, there are still some patients, 2 to 10%, who have re‐bleeding from recurrent esophageal varices even after repeated sclerotherapies 56,57 . In particular, huge esophageal varices fed by a large left gastric vein without the development of extravariceal portosystemic shunt are the typical sclerotherapy resistant varices 58 .…”

“…In practice, refusal proved to be a more frequent problem than expected, which complicated the interpretation of results. However, this problem is not unique to the pre-randomization method and similar difficulties can occur with conventional randomization [3,5,34-36]. Otherwise, no problems were encountered and our general experience was that the method worked well.…”
