Promoting Judicious Primary Care Referral of Patients with Chest Pain to Cardiology: A Quality Improvement Initiative

Abstract: Objective To decrease referrals to cardiology of patients ages 7 to 21 years with low-probability cardiac pathology who presented to primary care with chest pain by 50% within 24 months. Study Design A multidisciplinary team designed and implemented an initiative consisting of 1) a decision support tool (DST), 2) educational sessions, 3) routine feedback to improve use of referral criteria, and 4) patient family education. Four pediatric practices, comprising 34 pediatricians and 7 nurse practitioners, were in… Show more

“…The ACCU consists of 26 private and shared beds located within a free-standing quaternary Children’s Hospital in Washington, DC. 4 , 5 The ACCU is not a closed cardiac unit, though cardiology patients and cardiac surgery patients are solely admitted to the unit. Nephrology patients are preferentially admitted to the ACCU, and other services could be admitted during high census times when their traditional units are full.…”

“…In addition, feedback and support from medical advisors are provided to the FPs for “non-standard” patients’ registration and prioritisation (i.e., patients intended to be referred, but who do not meet the listed reasons for consultation), and administrative conformity and clinical validation processes conducted by nurses and/or administrative agents [ 15 ]. Internationally, other models incorporate similar features in centralized referral mechanisms to refer patients from primary to specialized health care [ 19 22 ].…”

“…As the authors alluded to, common paediatric cardiac complaints are often not caused by cardiac disease. 4,[11][12][13][14] The issue of subspecialty referral is complicated as for some the reason for referral might not be related to their illness elements. In other words, the drive for referral or testing is not related to patient factors like history or examination but rather to parents'/providers' anxiety and fear of litigation.…”

“…From our previously validated red-flag criteria for referral of children presenting with chest pain, we created a decision support tool. 4,12 Our healthcare quality improvement initiative utilised the decision support tool that was embedded within the electronic health care at primary care paediatrician offices. After multiple interventions, the utilisation of the decision support tool increased from 16 to 68% with simultaneous reduction in low probability referrals from 17 to 5%.…”
