Project BOOST: Effectiveness of a multihospital effort to reduce rehospitalization

Abstract: BACKGROUND: Rehospitalization is a prominent target for healthcare quality improvement and performance-based reimbursement. The generalizability of existing evidence on best practices is unknown.
