Progressive multifocal leukoencephalopathy in patients with sarcoidosis

Abstract: PML during sarcoidosis is often misdiagnosed. It is not associated with severe CD4 lymphocytopenia. Fatality rate is high in comparison with PML associated with other conditions. Interrupting immunosuppression remains the mainstay of treatment.

“…This was despite the fact that his brain MRI lacked contrast enhancement, which is very atypical for neurosarcoidosis. Very importantly, sarcoidosis is a known risk factor for PML and frequently causes a delay in diagnosis [5] . This was definitely the case for patient D, who was treated for neuro-sarcoidosis for months before we decided to do a brain biopsy.…”

“…When clinical deterioration progresses despite intensive immunemodulating treatment in neurosarcoidosis, a progressive multifocal leukoencephalopathy should be considered. This potentially fatal treatment complication in immunocompromised patients can be easily missed (131).…”

“…5 In rare cases, patients with sarcoidosis developed PML with-out immunosuppressive therapies. [6][7][8] These cases suggest that systemic sarcoidosis itself might cause PML. Although not fully understood, the pathogenesis of PML in sarcoidosis might be explained by a regulatory T cell/ Th1 imbalance and redistribution of T cells to the sites of disease involvement.…”

“…A study of 58 HIV‐negative patients with PML showed that 9% had sarcoidosis as the underlying disease . In rare cases, patients with sarcoidosis developed PML without immunosuppressive therapies . These cases suggest that systemic sarcoidosis itself might cause PML.…”
