Progressive Monoarticular Inflammatory Arthritis Following Influenza Vaccination

Abstract: Musculoskeletal injury is an uncommon but usually self-limited complication of vaccine administration. We present a case of progressive inflammatory monoarthritis of the shoulder characterized by bone erosion, bursitis, and severe synovitis caused by an influenza vaccine administered to the ipsilateral deltoid region. Clinical symptoms began within 2 hours of vaccination, with progressive decline in function over 6 weeks. Magnetic resonance imaging examinations performed 5 months apart demonstrated progressive… Show more

“…SIRVA is a rare condition following vaccine injection within 48 hours which is usually related to influenza vaccinations [ [2] , [3] , [4] , [5] , [6] ]. In this case, the patient developed right shoulder pain 72 hours following a Sinovac COVID-19 vaccine injection.…”

“…If the symptoms do not improve, a shoulder injury related to vaccine administration (SIRVA) will be considered, perhaps related to an improper injection technique. SIRVA is a rare condition, usually related to an influenza vaccination [ [2] , [3] , [4] , [5] , [6] ]. The pathogenesis of this condition is an immune response to intracapsular inoculation after vaccination [ 7 ], which is usually treated with medications such as oral non-steroidal anti-inflammatory drugs [ 6 ] or an intrabursal steroid injection [ 4 , 5 ].…”

“…These symptoms normally improve in a few days. If the clinical symptoms persist for more than 2–3 days, a shoulder injury related to vaccine administration (SIRVA) should be considered [ [2] , [3] , [4] , [5] , [6] ]. A SIRVA usually occurs due to an incorrect injection technique such as inappropriate needle direction or depth of penetration.…”

“…Local symptoms are mainly things such as pain, redness, warmth, swelling, itching and/or bruising, while systematic symptoms involve things such as skin rash, gastrointestinal side effects (nausea, vomiting and/or diarrhea), headache, fever, malaise, chills, and joint or muscle pain [ 1 , 2 ]. If these clinical symptoms continue longer than few days, a shoulder injury related to vaccine administration (SIRVA) should be considered [ [3] , [4] , [5] , [6] , [7] ]. A SIRVA is usually the result of an incorrect vaccine injection technique, usually from one (or more) of three things: using the wrong landmark or incorrect needle direction and/or depth of needle penetration.…”
