Progression of coronary artery calcification seems to be inevitable, but predictable - results of the Heinz Nixdorf Recall (HNR) study

Abstract: AimCoronary artery calcification (CAC), as a sign of atherosclerosis, can be detected and progression quantified using computed tomography (CT). We develop a tool for predicting CAC progression. Methods and resultsIn 3481 participants (45–74 years, 53.1% women) CAC percentiles at baseline (CACb) and after five years (CAC5y) were evaluated, demonstrating progression along gender-specific percentiles, which showed exponentially shaped age-dependence. Using quantile regression on the log-scale (log(CACb+1)) we dev… Show more

“…In fact, 68.1 % of participants lie within the band, 19.4 % above (fast progress), and 12.5 % below (slow progress) [19]. Using the deviation in individual follow-up and baseline CAC percentile, they established a new definition of CAC progression, which may overcome limitations of previous approaches [19]. While today no accepted gold standard for the measurement of CAC progression exists and each model may overestimate the progression according to CAC group at baseline, currently authors tend to confirm their findings with at least another model.…”

“…A predefined acceptance band nominally (at random assignment) covered 20 % of observed values around the individually predicted value at follow-up. In fact, 68.1 % of participants lie within the band, 19.4 % above (fast progress), and 12.5 % below (slow progress) [19]. Using the deviation in individual follow-up and baseline CAC percentile, they established a new definition of CAC progression, which may overcome limitations of previous approaches [19].…”

“…7 Approximately half of the obstructive lesions visualized by invasive coronary angiography or coronary computed tomography angiography (CCTA) are a major cause of ischemia. This process is independent of the degree of luminal stenosis in case of necrotic core, positive remodeling, or spotty calcifications revealed by IVUS and CCTA.…”
