BackgroundInpatient mental health care is a challenging component of the mental health services system, with frequent reports of negative and coercive experiences and doubts about its therapeutic value. As such, alternative approaches for individuals experiencing a mental health crisis are highly desirable. This research aimed to identify models which offer an alternative to standard inpatient mental health care across all age groups, both nationally and internationally, and to develop a typology for these alternative models.MethodsA dual literature search and expert consultation research methodology was adopted to identify relevant models. Three typologies of models were developed according to age group and acuity, including: alternatives to standard acute inpatient services for adults; alternatives to longer-stay inpatient services for adults, including rehabilitation and forensic inpatient services; and alternatives to standard inpatient services for children and young people.ResultsWe identified an array of service models in each typology, some in community settings, some hospital-based and some working across settings. Models varied greatly in characteristics, extent of implementation and supporting evidence.ConclusionsThrough this mapping exercise, we have developed three novel typologies of alternatives to standard inpatient care. A range of community-based, hospital-based and cross-setting approaches were identified. The identification of services providing inpatient care in a substantially different way to the standard suggests that some improvements could be provided within existing structures. Potential inequities in access to alternatives were identified for certain groups, such as people who are compulsorily detained, younger children and young people transitioning between children’s and adult services. These typologies can inform future description, evaluation and comparison of different service models. This research also yields some key considerations for the design, development and implementation of alternative mental health service models and service arrays.

Les trois Instituts universitaires en santé mentale (IUSM Douglas, de Montréal et de Québec) et le Centre hospitalier universitaire de Sherbrooke ont déposé un mémoire au Forum national de consultation sur le prochain plan d’action en santé mentale (PASM) 2014-2020, organisé en janvier 2014 par le ministère de la Santé et des Services sociaux (MSSS). À titre d’acteurs-clés du réseau de la santé mentale, ils commentent chacun des éléments présentés dans le document de consultation.Ils considèrent que les orientations proposées sont en continuité avec le PASM 2005-2010 et les thématiques présentées reflètent bien les enjeux actuels. Ils suggèrent des principes plus explicites quant à l’organisation de services souhaitée, soit l’exercice de la pleine citoyenneté, une organisation en réseaux intégrés de services, la performance, l’amélioration continue et l’innovation, ainsi qu’une vision globale et intégrée de la santé. La complexité des problématiques actuelles commande une offre de services souple, complémentaire et en continuité, particulièrement pour les jeunes, les autochtones et les personnes présentant des troubles concomitants. Ils insistent donc sur l’importance d’agir en prévention, d’offrir des programmes d’intervention précoce et de consolider le soutien offert aux omnipraticiens et aux professionnels de la première ligne. Ils rappellent cependant de ne pas négliger l’offre de services spécialisés en ambulatoire et en hospitalisation. Les services offerts dans la communauté doivent être structurés autour des niveaux de soutien variés, tels que le SIV et le SIM, mais aussi autour de programmations spécialisées disponibles dans les services de consultations externes des hôpitaux. Aussi, la consolidation des services surspécialisés, l’enseignement et la recherche sont à inclure dans le prochain plan d’action en santé mentale. Finalement, une perspective de santé globale doit dépasser le cadre du MSSS pour devenir un engagement gouvernemental interministériel, s’appuyant sur une vision de santé publique de la santé mentale qui examine les conditions de santé de la population et tient compte des déterminants sociaux. Ce plan sera la base d’un réel soutien à l’exercice de la pleine citoyenneté et à la lutte à la stigmatisation, en collaboration avec les personnes utilisatrices de services et leurs proches.Goal: Quebec’s three mental health university institutes (DMHUI, IUSMM and the IUSMQ) and the Centre hospitalier universitaire de Sherbrooke submitted a statement to the provincial consultation forum on the 2014-2020 Mental Health Action Plan (MHAP), which was held in January 2014 and organized by the Ministère de la Santé et des Services sociaux (MSSS). Background: Mindfulness-Based Interventions (MBI) have revealed efficacies in lowering psychological problems including depression and anxiety in a variety of clinical settings, including psychosis. Aim: The current study aimed to assess the effect of MBI on recent-onset psychosis for patients with schizophrenia. Methods: A randomized control trial research design recruited 69 patients with schizophrenia and randomly assigned them to a MBI group (35 patients) who received seven 90-minutes weekly group sessions and to a treatment-as-usual (TAU) group (34 patients). After informed consent, all patients were assessed using the Positive and Negative Syndrome Scale (PANSS), the Recovery Assessment Scale-Domains and Stages (RAS-DS), and the Five Facet Mindfulness Questionnaire (FFMQ) at baseline and post-intervention. Results: No significant difference was found between MBI group and TAU group regarding sociodemographic and clinical characteristics, studied domains and total scores of studied variables before the intervention. After application of the intervention, there was a significant difference between TAU group and MBI group in domains of positive psychosis symptoms, negative psychosis symptoms general psychopathology, PANSS total score (Z= 2.06, 3.11, 2.16 and 2.46 respectively). Besides, there was a significant difference between TAU group and MBI group regarding RAS-DS total score and FFMQ total score (Z= 4.19 and 4.78 respectively). Conclusion: This study preliminarily recognized prompt and transient benefits of MBI as a third-wave intervention on recent-onset psychosis for patients with schizophrenia. These types of intervention are recommended for nurses as a complementary treatment for positive and negative symptoms and enhancing recovery among schizophrenia patients.
