Programa de atenção ao idoso: relato de um modelo assistencial

Abstract: Este artigo trata do relato de um projeto de extensão universitária que propõe um modelo de atenção ao idoso. O objetivo do programa é oferecer assistência a idosos de baixa renda residentes no município de Ijuí-RS, Brasil, em situação de fragilidade e com risco de médio a alto de internação ou reinternação hospitalar.
