Prognostic Value of Thyroid Hormone FT3 in General Patients Admitted to the Intensive Care Unit

Abstract: Low plasma triiodothyronine (T3) concentration indicates nonthyroidal illness syndrome (NTIS), which might be associated with a poor outcome in patients in the intensive care unit (ICU). This study evaluated the relationship between NTIS and prognostic indicators in patients admitted to the ICU and examined the fT3 cut-off points that could be associated with 28-day mortality. This prospective observational study included patients admitted to the ICU of The Third Hospital of Hebei Medical University from Febru… Show more

“…[ 11 12 13 14 15 ] Persistent low T3 levels may have a deleterious effect on clinical recovery as seen among critically sick hospitalized children and adults. [ 1 16 ] Sick adults admitted in ICU showed increased risk of mortality with low T3 levels,[ 17 18 19 ] as also seen in the present study. However, most patients with low T3 including children recovered spontaneously without the need for thyroxine (LT4) supplementation.…”

“…Low FT3 serum values were reported to be independently associated with 28-day mortality in a prospective observational study focused on a large cohort of patients (n. 305) admitted to the ICU [41]. The occurrence of NTIS in COVID-19 patients has been reported in retrospective studies performed in small cohort of patients [42,43].…”

“…NTIS is considered as an adaptive response to acute illness, occurring during the fasting state in healthy individuals, as well as to other systemic diseases, including cardiovascular and infectious diseases [15][16][17]. Previous studies have shown that low T3 concentrations are usually associated with an adverse prognosis in patients with critical illness [18,19]. However, other studies have shown that decreased TSH concentrations were independently associated with mortality in patients with liver failure [20] or older adults in the community [21].…”
