Colorectal cancer (CRC) is one of the most common malignant diseases in the world. Presently, the most widely used staging system for CRC is the tumor nodes metastasis classification system, which classifies patients into prognostic groups according to the depth of the primary tumor, presence of regional lymph node (LN) metastases, and evidence of distant metastatic spread. The number of LNs with confirmed metastasis is related to the severity of the disease, but this number depends on the number of LNs retrieved, which varies depending on patient age, tumor grade, surgical extent, and tumor site. Numerous studies and a recent structured review have demonstrated associated improvements in the survival of CRC patients with increasing numbers of LNs retrieved for examination. Hence, the impact of lymph node ratio (LNR), defined as the number of metastatic LNs divided by the number of LNs retrieved, has been investigated in various malignancies, including CRC. In this editorial, we review the literature demonstrating the clinicopathological significance of LNR in CRC pati ents. Some reports have indicated the advantage of considering the LNR compared to the number of LNs retrieved and/or LN status. When the LNR is taken into consideration for survival analysis, the number of LNs retrieved and/or the LN status is not always found to be a prognostic factor. The cutoff points for LNRs were proposed in numerous studies. However, optimal thresholds for LNRs have not yet received consensus. It is still unclear whether the LNR has more prognostic validity than N stage. For all these reasons, the potential advantages of LNRs in the staging system should be investigated in large prospective data sets.

Original Article ÖZET Amaç: Bu çalışma radikal olarak rezeke edilmiş nonmetastatik kolorektal kanserlerde lenf nodu oranın (LNO) prognostik önemini belirlemek için yapılmıştır. Gereç ve Yöntemler:Servisimizde nonmetastatik kolorektal kanser nedeni ile opere edilen 280 hastanın karakteristik ve patolojik özellikleri retrospektif olarak incelenerek kaydedildi. Metastatik lenf nodlarının çıkarılan toplam lenf noduna oranının 1 /12, ¼, ½ ve 1 arasında olmasına göre hastalar 4 gruba ayrıldı. Demografik özelliklerinin dağılımları ve ihtimal tabloları ANOVA ve ki-kare testi ile yapıldı. LNO, N evresi ve TNM evresine göre kümülatif sağkalım oranlarını hesaplamak için Kaplan Meier metodu kullanılırken, gruplar arasında sağkalım farkını hesaplamak için Log rank testi kullanıldı Bulgular: Metastatik lenf nodu sayısı ile LNO arasında istatistiksel olarak anlamlı bir korelasyon oranı mevcuttu (Pearson korelasyon katsayısı (r)=0.674, p<0.001). Hastaların 173 tanesinde (%61.8) 12'den daha az lenf nodu çıkarılmıştı. Değerlendirmede 166 hasta (%59.3) N0, 79 hasta (%28.2) N1 ve 35 hasta (%12.5) N2 olarak tespit edildi. LNO göre oluştu-rulan 4 gruptaki hasta sayıları sırası ile 174 (%62.1), 72 (%25.7), 22 (%7.9) ve 12 (%4.3) idi. LNO1 grubunda 5 yıllık sağkalım oranı %82,8 iken, LNO2 grubunda %69.4, LNO3 grubunda %58.3 ve LNO4 grubunda %50 olarak tespit edildi. Bu gruplar arasındaki fark sağkalım süresi ve oranları açısın-dan istatistiksel olarak anlamlı idi (p=0.001).Sonuç: Doğru evreleme için yetersiz lenf nodu çıkarılmış hastalarda LNO adjuvan tedaviye karar vermede ve prognozu kestirebilmede hekimlere yol gösterecek önemli parametrelerden biri olabilir. ABSTRACT Objective: This study was designed to assess the prognostic value of lymph node ratio (LNR) in patients with nonmetastatic colorectal cancer treated with radical resection. Material and Methods:The characteristic and pathologic parameters of 280 patients operated on for nonmetastatic colorectal cancer at our service were recorded retrospectively. Patients were stratified into 4 groups according to the LNR quartiles of 1 /12, ¼, ½ and 1. Distribution of demographic data and probability tables were made by ANOVA and χ 2 tests. The cumulative survival ratios for LNR, N stage and TNM stage were calculated by the Kaplan Meier method and survival difference between groups were calculated by the log rank test.Results: There was a statistically significant correlation ratio between metastatic lymph node count and LNR. In 173 of all patients (61.8%) fewer than 12 lymph nodes were harvested. One hundred sixtysix (59.3%) patients were evaluated as N0, 79 (28.2%) were N1 and 35 (12.5%) were N2. The number of patients in 4 groups of LNR were 174 (62.1%), 72 (25.7%), 22 (7.9%) and 12 (4.3% ) respectively. Five year survival ratios were 82,8% in LNR1, 69.4% in LNR2, 58.3% in LNR3 and 50% in LNR4. ÖZETAmaç: Kolorektal kanserli hastalarda, standart faktörlerin yanı sıra, tutulu lenf nodlarının çıkarılan lenf nodlarına oranının (LNR), sağkalımla olan ilişki-sinin belirlenmesi hedeflenmiştir.Yöntemler: Ocak 2003-Mart 2010 yılları arasında kolorektal kanser nedeniyle ameliyat edilen ve istenen kriterleri sağlayan 72 hastanın dosyaları, T, N ve M durumu ve evreleri, lenfovasküler ve perinöral invazyon varlığı, çıkarılan lenf nodu sayısı, tutulu lenf nodu sayısı, lenf nodu kapsül invazyonu varlığı ve LNR açısından incelenmiş ve bu parametrelerin sağkalımla ilişkileri retrospektif olarak araştırılmıştır.Bulgular: Kolorektal kanserli hastaların sağkalımlarıyla, LNR, lenf nodu evresi, tutulu lenf nodu sayısı, TNM evresi, PNI ve lenf nodu kapsül invazyonu varlığının korelasyon gösterdiği belirlenmiştir.Sonuç: LNR ve lenf nodu evresi, incelenen parametreler arasında sağkalımla en yüksek ilişkili parametreler olarak bulunmuştur. The variables such as T, N and M stage, lymphovascular and perineural invasion, number of retrived lymph nodes, number of metastatic lymph nodes, lymph node capsule invasion, LNR and their effects on overall survival were analysed.Results: LNR, lymph node stage, number of metastatic lymph nodes, TNM stage, perineural invasion and lymph node capsule invasion had significant effects on overall survival.Conclusion: LNR and lymph node stage were the most significant variables associated with overall survival.
