The relation between chronic obstructive pulmonary disease (COPD) and chronic kidney disease (CKD) has been largely undescribed and the occurrence and frequency of anemia in COPD has rarely been studied, so the aim of this study is: to estimate the frequency of undiagnosed CKD among COPD patients and to verify whether concealed CKD is prevalent in COPD population, and to assess the frequency of anemia in COPD patients and its relation to the severity of the disease. Patients and methods: this study included 100 stable COPD patients with various degrees of severity. They were divided into 2 groups: group (І): patients with mild to moderate severity of COPD (n=43) and group (П): patients with severe to very severe COPD (n= 57). All studied patients were subjected to: pulmonary function tests, complete blood count, erythropoietin level measurement, glomerular filtration rate measurement, and arterial blood gases analysis. Results: there was a statistically non-significant increased frequency of concealed chronic renal failure (CRF) in patients of group (П) than that of group (І). But a significant increased frequency was seen in overt CRF in patients of group (П) than that of group (І) with a statistically significant positive correlation between COPD severity and renal function reduction . As regards the frequency of anemia, there was a statistically significant high frequency of anemia in patients of group (П) than that of group (І) with a statistically very highly significant negative correlation between COPD severity and hemoglobin level. Conclusions: 1) CRF occurs in high rates in COPD patients and it is related to the disease severity. 2) Anemia is a frequent co-morbidity among COPD patients especially in those with severe disease.

Background. The evaluation of chronic obstructive pulmonary disease (COPD) has been shifting from spirometry to focus on the patients' overall health. Despite the existence of many COPD prognostic scales, there remains a large gap for improvement, in particular a scale that incorporates the current focus on overall health. Methods. We proposed a new prognostic scale (the COPD Prognostic Score) through discussion among the authors based on published studies. Validation was retrospective, using data from the National Emphysema Treatment Trial. Results. The scores ranged from 0-16, where 16 indicated the poorest prognosis. We assigned 4 points each for forced expiratory volume in one second (%predicted), the modified Medical Research Council dyspnea scale, and age; 2 points for the hemoglobin level; and one point each for decreased activity and respiratory emergency admission in the last two years. The validation cohort included 607 patients and consisted of 388 men (73.9%) and 219 women (36.1%), mean age 67 ± 6 years and an average forced expiratory volume in one second (% predicted) of 27 ± 7%. A one-point increase in the score was associated with increased all-cause death, with a hazard ratio of 1.28 (95%CI: 1.21-1.36. P < 0.001). The areas under the receiver operating characteristic curves for two-year and five-year all-cause death for the new scale were 0.72 and 0.66, respectively. These values were higher than those given by the body mass index, airflow obstruction, dyspnea, and exercise capacity (BODE) index and age, dyspnea, airway obstruction (ADO) index. Conclusion. The preliminary validation for a new COPD prognostic scale: the COPD Prognostic Score was developed with promising results thus far. Above mentioned 16-point score accurately predicted 2-year and 5-year all-cause mortality among subjects who suffered from severe and very severe COPD.

Background and Aims: Unlike classically described polycythemia, anemia is found to be more prevalent in patients with chronic obstructive pulmonary disease (COPD).Anemia increases the cost of hospital stay and causes an increased risk of adverse outcomes including death in COPD patients. This study was done to find the prevalence of anemia in COPD patients, the factors associated, and the outcomes of anemic COPD.Methods: It was a quantitative, descriptive-analytical, and cross-sectional study conducted in Tribhuvan University Teaching Hospital's medical wards and the Emergency Room from September 2019 to September 2020. A simple random sampling method was used. Clinical information was obtained, and patients were followed up 3 months after discharge to document the number of exacerbations and deaths if present. Results:The patients in our study had a mean age of 70.80 ± 11.16 years. Most were female. Most (85.5%) had a history of exposure to firewood smoke. Twenty-three percent of the patients had anemia and these patients had significantly greater mortality 3 months postdischarge. Middle-old and old were more likely to have anemia with odds ratio (OR) of 2.55 (confidence interval [CI]: 0.48-13.5) and 13.6 (CI: 1.12-24.2), respectively. Current smokers had less likelihood of having anemia (OR: 0.05, CI: 0.006-0.49). Multivariate analysis showed that age, sex, and smoking status were significant determinants of anemia in COPD. There was no association between anemia and duration of hospital stay. However, mortality was higher at 3 months in COPD patients with anemia (p < 0.001). Conclusion:In COPD patients, anemia is prevalent comorbidity that is significantly linked to higher mortality but not to exacerbations. It is unknown, though, if treating anemia in COPD patients will affect the patient's outcome. Additional research in this area may be possible.
