Prognostic value of testosterone castration levels following androgen deprivation and high-dose radiotherapy in localized prostate cancer: Results from a phase III trial

Abstract: Background/objective: The optimal prognostic value of testosterone following androgen deprivation therapy (ADT) is controversial. We studied the effect of serum testosterone levels on clinical outcome in localized prostate cancer (PCa) treated with ADT and high-dose radiotherapy (HRT). Patients and methods: The DART01/05 trial randomized 355 men with intermediate and high-risk PCa to 4 months of ADT plus HRT (STADT, N = 178) or the same treatment followed by 24 months of ADT (LTADT, N = 177). This study includ… Show more

“…The use of testosterone treatment in patients with previous prostate cancer is highly debated as testosterone has been suggested to increase prostate cancer risk and disease progression (Morgentaler & Traish 2009). In contrast, a short time to recovery of endogenous serum testosterone is not considered a risk factor for relapse (Roy et al 2020, Zapatero et al 2021. Randomized controlled trials of testosterone treatment for the recovery of serum testosterone levels are missing.…”

“…The number of elderly men will substantially increase in the coming decades and hence their well-being is of general concern for public health [49]. Moreover, testosterone levels may also play an important role in the development of prostate cancer [53], and they have potential implications for the prognosis of prostate cancer patients [54].…”

“…On the other hand, our study included patients who received BT boosts using a fraction dose of approximately 10 Gy. Giving that, the probability of PSA bounces increases with fraction dose [ 23 ] and perhaps this index could prove to be useful in patients receiving a single-fraction BT boost with a higher fraction dose. PSA bounce is challenged by a few important limitations related to definition of the index.…”
