Prognostic Value of Serum Concentration of Heart-Type Fatty Acid–Binding Protein Relative to Cardiac Troponin T on Admission in the Early Hours of Acute Coronary Syndrome

Abstract: Background: Heart-type fatty acid-binding protein (H-FABP) is proposed as an early biomarker for acute myocardial infarction (AMI), but its prognostic value is unclear in acute coronary syndrome (ACS). We evaluated the prognostic value of the H-FABP concentration relative to cardiac troponin T (cTnT) in the early hours of ACS. Methods: Serum concentrations of H-FABP and cTnT were measured on admission in 328 consecutive patients hospitalized for ACS within 6 h after the onset of chest pain [AMI, 241 (73.5%) pa… Show more

“…64 In patients with ACS, serum H-FABP is an independent predictor of cardiac events (cardiac death or nonfatal MI) on intermediate followup (12 months) and has a greater predictive capacity for cardiac events than cTnT. 66,67 H-FABP may also be used as a reliable marker for the evaluation of the severity of congestive HF. 68 Its levels have been positively correlated with BNP levels in patients with ADHF before and after treatment.…”

“…One earlier study with the use of the same technology demon strated a 100% sensitivity for H-FABP and higher AUC (0.998) at cut-point of 5.08 ng/mL within 6 hours from the onset of chest pain (10). On average, the sensi tivity of H-FABP assays for the early diagnosis of AMI reaches 90% at 5-6 ng/mL as cut-off value (6)(7)(8)11).…”

“…Importance of biomarkers, both in diagnosis and prognosis of ACS is now well established [2] and proved superior to electrocardiographic guidance alone [8]. Cardiac troponins remain the cornerstone in the risk stratification of patients with suspected ACS [14].…”

“…Heart-type fatty acidbinding protein (h-FABP) is a low molecular weight (14.5 kDa) protein, which contains 132 amino acid residues [7]. h-FABP is found in abundance in cardiomyocytes but is also expressed (to a lesser extent) in skeletal muscle, distal tubular cells of the kidney, specific parts of the brain, lactating mammary glands and placenta [8]. h-FABP binds two molecules of fatty acids and is involved in the delivery of fatty acyl coenzyme A, which is actively included in oxidation processes, which generate energy in the mitochondria.…”
