Prognostic value of preoperative circulating tumor cells counts in patients with UICC stage I-IV colorectal cancer

Abstract: Colorectal cancer (CRC) is one of the leading causes of cancer death worldwide. There is an urgent need to identify prognostic markers for patients undergoing curative resection of CRC. The detection of circulating tumor cells in peripheral blood is a promising approach to identify high-risk patients with disseminated disease in colorectal cancer. This study aims to evaluate the prognostic relevance of preoperative CTCs using the Cellsearch® system (CS) in patients, who underwent resection with curative intent… Show more

“…The clinical utility of identifying circulating tumour cells in colorectal cancer patients has already been described by several authors [ 15 , 22 , 23 , 25 , 27 , 38 , 39 ]. However, current protocols for the isolation and characterisation of CTCs are mainly limited to the microscopic evaluation of one or two markers, compromising the ability to identify different CTC subpopulations.…”

“…We defined CTCs as cells expressing cytokeratin (either bound to IsoFlux TM beads or not, as we pre-treated samples with papain to facilitate their analysis). Cut-off values of CTCs in non-metastatic colorectal cancer have been defined as between 1 and 5 cells in different studies [ 23 , 25 , 38 , 39 ]. However, as our methodology is more sensitive and efficient, we detected more CTCs per patient, and thus, cut-off values could be slightly elevated.…”

“…In the last decades, biomarkers for micrometastasis like circulating and disseminating tumor cells (CTC and DTC) were identified. Tumor dissemination in peripheral blood and bone marrow proved to be associated with shorter OS and PFS in different solid tumors and PDAC [ 4 , 5 , 18 ]. DTCs stay dormant in the bone marrow and recirculate into various sites causing metastasis or even tumor recurrence [ 19 ].…”

“…Plasma DNA can be analyzed by approaches targeting specific tumor-associated genes (e.g., mutations in the EGFR gene in non-small cell lung carcinoma (NSCLC)) or non-targeted screening approaches such as array CGH, whole-genome sequencing, or exome sequencing) [ 25 , 26 ]. In general, targeted approaches have higher analytical sensitivity than non-targeted approaches, but there are strong efforts to improve the detection limits of non-targeted approaches [ 27 , 28 ].…”

“…According to Bork et al, the detection of CTCs in nodal negative CRC (UICC stage I–II) is as low as 9% [ 34 , 35 ]. In another study, the detection of CTCs using Cellsearch ® in CRC (UICC stage I–IV) was 45% [ 26 ]. Therefore, the utility of CTC-based screening using Cellsearch ® is still rather challenging and still not applicable ( Table 1 ).…”
