Prognostic value of myocardial fibrosis in severe aortic stenosis: study protocol for a prospective observational multi-center study (FIB-AS)

Abstract: Background: Adverse cardiac remodeling with a myocardial fibrosis as a key pathophysiologic component may be associated to worse survival in aortic stenosis (AS) patients. Therefore, with the application of advanced cardiac imaging we aim to investigate left ventricular myocardial fibrosis in severe AS patients undergoing aortic valve replacement (AVR) and determine its impact with post-intervention clinical outcomes. Methods: In a prospective, observational, cohort study patients with severe AS scheduled eith… Show more

“…Sample size calculations were performed for this observational study using the Cox proportional hazards model, as reported previously. 27 The prevalence of HF patients with a BI score <85, 1-year mortality rate, and the hazard ratio between BI score and all-cause mortality in elderly HF patients. The dotted line represents a hazard ratio (HR) of 1.0, the purple line represents HRs, and the shaded area represents 95% confidence intervals.…”

“…This multifunctional molecule is validated for therapeutic intervention and for monitoring replacement myocardial fibrosis secondary to heart damage. Replacement myocardial fibrosis has prognostic value usually associated with risk factors of poor cardiac recovery, [19][20][21] that strongly affects heart function by overexpression and deposition of extracellular matrix pro-fibrotic components, mainly collagen I. However, therapies to directly target excessive fibrosis remain limited.…”

“…The study included patients with severe symptomatic AS that were scheduled for surgical AVR according to the current treatment recommendations [13]. The trial design and baseline characteristics of the patients have been described previously [23,24]. The study was approved by the local biomedical research ethics committee (number: 158200-18/9-1014-558) and conformed to the principles of the Helsinki Declaration.…”
