Prognostic value of fibrinogen to albumin ratios among critically ill patients with acute kidney injury

Abstract: Fibrinogen to albumin ratios (FAR) have shown to be a promising prognostic factor for improving the predictive accuracy in various diseases. This study explores FAR's prognostic significance in critically ill patients with acute kidney injury (AKI). All clinical data were extracted from the Multiparameter Intelligent Monitoring in Intensive Care Database III version 1.4. All patients were divided into four groups based on FAR quartiles. The primary endpoint was in-hospital mortality. A generalized additive mod… Show more

“…As reported previously, elevated FPR and AFR better predict overall survival in various diseases than levels of albumin, pre‐albumin, and fibrinogen alone 10,14,15 . Indeed, despite plasma fibrinogen being the known component of the coagulation cascade, high fibrinogen levels were strongly associated with CVD mortality in inflammatory disorders by promoting the synthesis of proinflammatory cytokines 16,17 .…”

“…LC-MS procedures have however not been established for clinical routine use on a daily basis yet. Xia and colleagues [16] finally analyzed the predictive role of the fibrinogen level to the albumin level ratio (FAR) in more than 5,000 critically ill AKI subjects. The FAR had been evaluated in other diseases also (e.g., malignant disorders [17][18][19], myocardial infarction [20]).…”

“…The problem with such an approach is often the lack of control groups and/or the lack of comparisons with other, already established AKI biomarkers. For instance, the following biomarkers have been shown to predict the mortality risk in AKI: NGAL, KIM-1 (both [ 43 ]), the fibrinogen-to-albumin ratio (FAR) [ 44 ], soluble suppression of tumorigenicity 2 (sST2) (circulating interleukin (IL)-33 receptor isoform [ 45 ]), and certain panels of metabolites [ 46 ]. A 2020 published study by Fiorentino [ 47 ] on the other hand identified urinary (TIMP-2) • (IGFBP7) (tissue metalloproteinase-2 and insulin-like growth factor-binding protein-7) of > 0.3 as dialysis predictive in septic AKI individuals.…”
