Elevated preoperative plasma level of endoglin has been associated with worse oncologic outcomes in various malignancies. The present large-scale study aimed to determine the predictive and prognostic values of preoperative endoglin with regard to clinicopathologic and survival outcomes in patients treated with radical cystectomy (RC) for nonmetastatic urothelial carcinoma of the bladder (UCB). We prospectively collected preoperative blood samples from 1036 consecutive patients treated with RC for UCB. Logistic and Cox regression analyses were undertaken to assess the correlation of endoglin levels with pathologic and survival outcomes, respectively. The AUC and C-index were used to assess the discrimination. Patients with adverse pathologic features had significantly higher median preoperative endoglin plasma levels than their counterparts. Higher preoperative endoglin level was independently associated with an increased risk for lymph node metastasis, ≥pT3 disease, and nonorgan confined disease (NOCD; all p < 0.001).Plasma endoglin level was also independently associated with cancer-specific and overall survival in both pre-and postoperative models (all p < 0.05), as well as with recurrencefree survival (RFS) in the preoperative model (p < 0.001). The addition of endoglin to the preoperative standard model improved its discrimination for prediction of lymph node metastasis, ≥pT3 disease, NOCD, and RFS (differential increases in C-indices: 10%, 5%, 5.8%, and 4%, respectively). Preoperative plasma endoglin is associated with features of biologically and clinically aggressive UCB as well as survival outcomes. Therefore, it seems to hold the potential of identifying UCB patients who may benefit from intensified therapy in addition to RC such as extended lymphadenectomy or/and preoperative systemic therapy.

• Leukemia-forming activity is enriched in endoglinexpressing AML and B-ALL blasts using a mouse xenograft model. • Inhibition of endoglin function with TRC105 reduces leukemia development and progression.Endoglin (CD105), a receptor of the transforming growth factor-b superfamily, has been reported to identify functional long-term repopulating hematopoietic stem cells, and has been detected in certain subtypes of acute leukemias. Whether this receptor plays a functional role in leukemogenesis remains unknown. We identified endoglin expression on the majority of blasts from patients with acute myeloid leukemia (AML) and acute B-lymphoblastic leukemia (B-ALL). Using a xenograft model, we find that CD105 1 blasts are endowed with superior leukemogenic activity compared with the CD105 2 population.We test the effect of targeting this receptor using the monoclonal antibody TRC105, and find that in AML, TRC105 prevented the engraftment of primary AML blasts and inhibited leukemia progression following disease establishment, but in B-ALL, TRC105 alone was ineffective due to the shedding of soluble CD105. However, in both B-ALL and AML, TRC105 synergized with reduced intensity myeloablation to inhibit leukemogenesis, indicating that TRC105 may represent a novel therapeutic option for B-ALL and AML. OBJECTIVES: This study focuses on the evaluation of the effectiveness of radical gastrectomy with D2 node dissection after neoadjuvant therapy in the patients with gastric cancer. Gastric cancer is a widespread type of cancer, and it is the third leading cause of death in the cancer patients. Metastases most often occur in the lymph nodes and therefore, in addition to gastrectomy, lymph node dissection is often performed in the patients. We are distinguishing between D1, 2 and 3 dissections. As with other cancers, the effectiveness of neoadjuvant therapy is being considered, which aims to improve the patient's prognosis and thus the 5-year survival rate. METHODS: Within the study, we evaluated a group of the patients diagnosed with gastric cancer (n=41). The average age of the patients was 62.3 years (20-72 years). 68.3 % (n=28) patients underwent neoadjuvant therapy before surgery, the remaining 31.7 % (n=13) underwent only radical gastrectomy with D2 lymphadenectomy. In all the cases, an open operational approach was implemented. CONCLUSION: Lymphadenopathy was found in 85.4 % of the patients. Complications occurred in both groups, but in the group with neoadjuvant therapy their share was lower (14.3 % vs 23.1 %), while in the group without neoadjuvant treatment the proportion of duodenal leaks was higher, as well as the number of reoperations. In total, an average of 30 ± 8 lymph nodes were harvested. A lower number of T3-4 cases was found in the neoadjuvant group (17.9 % vs 61.6 %), confi rming that the tumour size was signifi cantly smaller in the neoadjuvant group than in the group, who did not undergo it and underwent only surgical treatment. Relapse was found in 29.3 % of the patients after neoadjuvant treatment and in 38.5 % of the patients without neoadjuvant treatment. Also, mortality due to relapse was higher in the group without neoadjuvant treatment (30.8 % vs 21.7 %). The average survival was 25 months (Tab.
