Prognostic significance of preoperative serum CYFRA 21‐1 in patients with upper urinary tract urothelial carcinoma

Abstract: Objectives:The prognosis for upper urinary tract urothelial carcinoma is generally poorer than that for bladder cancer, and prognostic predictors with a high specificity for upper urinary tract urothelial carcinoma (UUT-UC) are needed to optimize treatment. In fact, the only preoperative predictor currently available is C-reactive protein. In the present study, we investigated the usefulness of pretreatment serum CYFRA 21-1 as a new prognostic predictor in UUT-UC. Methods: A total of 45 UUT-UC patients for who… Show more

“…Serum CYFRA 21–1 has been reported as a prognostic factor in patients with a variety of cancer types, including resectable NSCLC [39,40], biliary tract cancer [41], urothelial cancer [42], head and neck cancer [43], esophageal cancer [44], and cervical cancer [45]. …”

“…Finally, in the development of blood- or urine-based markers for upper tract urothelial carcinoma, C-reactive protein, Cytokeratin 19 fragment (CYFRA21-1), and humoral factors (alkaline phosphatase and white blood cell counts) have been identified to be able to independently predict overall survival in cancer patients [43], [44], [45]. Interestingly, in a previous study, a high level of MACC1 protein in the serum collected from pancreatic cancer patients has been detected and suggested to be correlated with lymph node metastasis, distant metastasis and a later TNM stage [21].…”

“…Finally, the serum concentration of CYFRA in the present case was elevated to 76.8 ng/ml (compared to a normal level of <3.5 ng/ml). CYFRA is usually considered a lung cancer-specific tumor marker, but Suyama et al [4] have reported that CYFRA is also a useful tumor marker for urothelial carcinoma. Therefore, we conclude that the primary lesion was a left urothelial carcinoma that invaded the inferior vena cava with tumor microemboli extending to both lungs through the right side of the heart.…”
