Prognostic significance of distal renal tubular acidosis in posterior urethral valve

Abstract: The prognostic significance of distal renal tubular acidosis (DRTA) in the development of overt nephropathy (ON) in children with posterior urethral valves (PUV) is not clear. This condition was studied prospectively in 22 children with posterior urethral valve (PUV), with normal renal function. Prior to surgery, the children with ON had a higher incidence of bilateral reflux (P=0.006), but the difference was not significant for age at surgery (P=0.31), duration of voiding symptoms prior to surgery (P=0.30), p… Show more

“…Written informed consent was obtained from the parents or legal guardians of the participants prior to enrolment, and assent was taken from the subjects. The study group selection was based on the reported high prevalence of idRTA in patients with PUV [15,16]. The exclusion criteria were chronic kidney disease (CKD) after the surgical intervention, an alternative etiology of short stature, follow-up time shorter than 1 year, or loss to follow-up.…”

“…Unilateral ureter obstruction (UUO) and other forms of obstructive renal disease lead to tubular remodeling and may be associated in some forms with transient or persistent forms of renal tubular acidosis, mostly distal renal tubular acidosis [63,64]. In a rat model of adult bilateral ureter obstruction (for 24 hrs), a strong decrease in the expression of some acid-base transporting protein such as NHE3 or NBCn1 was documented.…”
