Prognostic Nutritional Index is a Predictor of Free Flap Failure in Extremity Reconstruction

Abstract: The nutritional condition of patients is an important prognostic factor in various diseases. Free flap failure is a serious complication in patients undergoing free flap reconstruction, increasing morbidity and hospital costs. We evaluated the predictive factors, including the prognostic nutritional index (PNI), associated with free flap failure in extremity reconstruction. The PNI was calculated as follows: 10 × serum albumin (g/dL) + 0.005 × total lymphocyte count (per mm3), with a PNI <40 defined as low.… Show more

“…Prognostic nutritional index (PNI) is based on the patient’s serum albumin level and total lymphocyte count and has been proposed as a method of assessing the perioperative nutritional status, complications and mortality in patients with colorectal cancer [ 10 ]. Low PNI is associated with poor outcomes in patients with malignancy [ 10–14 ], free flap reconstruction [ 15 ], heart failure [ 16 , 17 ] and kidney transplantation [ 18 ]. However, to our knowledge, no study has evaluated the association between perioperative PNI and postoperative mortality in elderly burn patients.…”

“…Women may have a smaller vascular diameter than men so that anastomoses will be more difficult. [21] In this study, there was no difference in the risk of flap failure between men and women. These results are similar to free-style flaps meta-analysis.…”

“…The main indicator of the patient's protein status is the serum albumin level (10,11). Prognostic nutritional index calculated by serum albumin and lymphocyte is a useful biomarker for postoperative complications and systemic inflammation (12). PNI value has been shown to be an independent prognostic factor for predicting postoperative complications and survival in patients undergoing different tumor surgeries (13)(14)(15)(16).…”
