Prognostic Importance of Atrial Fibrillation in Implantable Cardioverter-Defibrillator Patients

Abstract: In the population currently receiving ICD treatment outside the setting of clinical trials, a large portion has either a history of AF or permanent AF. Both types of AF have prognostic implications for mortality and appropriate as well as inappropriate device discharge.

“…38, 39 Borleffs et al reported that patients with permanent AF, as compared with those without it, had a 1.7-fold risk of all-cause mortality and 2-fold risk of appropriate ICD shock during a mean 833-day follow-up after ICD implantation. 38 Similarly, the present study found that cAF was associated with a 2.9-fold increase in relative HR of FAE, defined as a composite of SCD, arrhythmic death, VT/VF and appropriate ICD therapy in group A and B patients. LV enlargement has also been reported to be a predictor of SCD in CHF, IHD with HFrEF and NIDCM patients.…”

“…21 Data were prospectively collected since 1996 on 913 consecutive patients receiving an ICD for primary or secondary prevention of SCD. At implantation, 73% of patients had no history of AF, 9% had a history of paroxysmal AF, 7% had a history of persistent AF, and 11% had permanent AF.…”

“…Prevalent AF is associated with an increased risk of SCD in patients postmyocardial infarction 20,29,30 and in women with coronary artery disease, 31 but the relationship of SCD to AF in patients with heart failure has been inconsistent. 24,25,27 Moreover, in patients with implantable cardioverter-defibrillators (ICDs), a history of AF, 32 persistent AF at the time of ICD implantation, 33 and permanent but not persistent or paroxysmal AF, 34 have an increased risk of appropriate device therapies for ventricular arrhythmias.…”
