Prognostic factors in primary central nervous system lymphoma

Abstract: Purpose of reviewPrimary central nervous system lymphoma (PCNSL) is a rare and aggressive extranodal diffuse large B cell lymphoma. Despite its apparent immunopathological homogeneity, PCNSL displays a wide variability in outcome. Identifying prognostic factors is of importance for patient stratification and clinical decision-making. The purpose of this review is to focus on the clinical, neuroradiological and biological variables correlated with the prognosis at the time of diagnosis in immunocompetent patien… Show more

“…A MMSE < 27 was associated with a poor prognosis for patients younger than 70 years-old in a phase 3 study [29] while a MMSE < 25 was correlated to a poor survival for PCSNL older than 60 years in a phase 2 study [30]. A large number of biomarkers have shown their interest or are still being studied to assess their prognostic impact, including biomarkers in cerebrospinal fluid and neuroimaging data [31 ▪ ,32 ▪ ].…”

“…In a population-based study by the French oculocerebral lymphoma network, PCNSL patients (n = 1002) treated according to current national guidelines had a median overall survival (OS) of 25 months and a 5-year survival rate of 38% [10] . Age is the main independent prognostic factor for PCNSL with better prognoses in younger patients: the 5-year progression-free survival rates were 61% and 28% in patients under or above the age of 60 years, respectively [11] . This suggests that clinicians should consider the impact of age on prognosis when choosing the optimal treatment for PCNSL patients.…”

“…Primary Central Nervous System Lymphoma (PCNSL) constitutes a distinct subset of lymphoma, representing 4% of all cerebral malignancies. Its prognosis is notably poorer and mortality rate signi cantly higher when compared to other lymphoma subtypes [1][2][3] [4]. The prevalence of PCNSL has been on an upward trajectory in recent years, a trend partially attributable to the global aging population [3] [5].…”
